Position Summary

The facility is seeking a Board Certified/Board Eligible Palliative Care Physician to join a dedicated and compassionate team in Shelton, CT.

This full-time, permanent opportunity offers a collegial practice environment with a strong emphasis on patient-centered care, symptom management, and interdisciplinary collaboration. Physicians will provide expert guidance in managing pain, complex symptoms, and end-of-life care for patients with serious illnesses.

Essential Position Responsibilities

  • Conduct comprehensive palliative assessments and develop individualized care plans.

  • Provide symptom management, pain control, and supportive care for patients with serious illnesses.

  • Collaborate closely with primary care providers, specialists, nurses, social workers, and chaplains to ensure holistic patient care.

  • Facilitate discussions with patients and families regarding goals of care, advanced directives, and end-of-life planning.

  • Maintain accurate and timely documentation in the electronic medical record.

  • Participate in team meetings, case reviews, and quality improvement initiatives.

  • Promote patient education, preventive health strategies, and community wellness efforts.

Qualifications

  • Education: MD or DO degree from an accredited institution.

  • Licensure & Certification: Board Certified/Board Eligible in Palliative Care or relevant specialty; eligible for Connecticut medical licensure.

  • Experience: Previous clinical experience preferred, but new graduates and residents are welcome to apply.
