The LPN Medication Care Manager is responsible for providing the highest quality care by administering medications and treatments in a safe, organized manner. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care in accordance with Sunrise policies and applicable laws and regulations to promote quality care for residents.

Essential Duties
As part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is a fundamental part of this job. The role includes, but is not limited to, the essential functions listed below:
Medication Administration
Provide the highest degree of quality care by administering medications and treatments in a safe, organized manner.
Review, read, notate, and initial the electronic health record to document and learn about residents.
Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse.
Administer, assist with, and observe medications and treatments for each resident using the MAR and the Six Rights of Medication Pass.
Ensure medications are passed according to times using a mobile medication cart.
Document and initial as medications are given and document refusals or missed doses.
Maintain confidentiality of resident information.
Report resident concerns and unavailable medication to the RCD or Wellness Nurse.
Restock medication cart after passes.
Assist in checking medication regardless of packaging system.
Assess residents to determine need for as-needed medication and document/report appropriately.
Count narcotics with another MCM or LCM each shift and report discrepancies.
Maintain and clean the Medication Room and carts; remove expired medications.
Follow refill process for medications.
Help residents maintain independence and promote dignity and safety, adhering to Sunrise Principles of Service.
Respond empathetically to residents, considering their communication patterns, history, and needs.
Practice standard care precautions of cleanliness, hygiene, and health.
Audit medication carts.
Resident Care
Notify RCD of resident and/or family concerns.
Attend and participate in daily Cross Over meetings facilitated by the LCM.
Conduct Service and Health Updates as directed by RCD.
Participate in the development of the Individualized Service Plans (ISP).
Transcribe orders.
Risk Management and General Safety
Ensure community compliance with occupational health and safety requirements; support Risk Management programs and policies.
Practice safety procedures at all times including PPE, fire extinguishers, SDS, and Lockout Tagout procedures.
Report all accidents/incidents and unsafe conditions immediately.
Keep cords, carts, equipment, and hazards clear of exits and in compliance with fire codes.
Comply with infection control techniques and hazardous materials policies.
Follow safe handling and disposal of bloodborne pathogens and infectious waste.
Report occupational exposures immediately.
Ensure oxygen tanks are stored safely and monitored according to prescribed levels.
Training and Team Contribution
Participate as a team member and contribute to team goals.
Demonstrate the Team Member Credo in daily interactions.
Serve residents and guests through Principles of Service.
Contribute to engagement programs and participate in surveys and improvement workshops.
Attend regular meetings and trainings as directed by the Supervisor/Department Coordinator.
May be designated as shift supervisor or supervise other MCMs.
Maintain required trainings to meet Sunrise standards and regulatory requirements.
Perform other duties as assigned.

Experience And Qualifications
LPN, LVN, or state/province-specific licensed nurse credential
