Job Overview The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe, organized manner. Responsibilities include but are not limited to:
Administration of medications
Documentation of medication administration
Conducting Senior Health Updates (SHUs)
Providing resident care while demonstrating the Mission of Sunrise Senior Living: "to champion the quality of life for all seniors" in accordance with federal, state/provincial, and local laws and Sunrise policies.
Responsibilities & Qualifications Essential Duties As part of the Sunrise team, supporting our Mission, Principles of Service, and Core Values is fundamental to this role. The unique responsibilities for this position include:
Provide quality care and services by administering medication and treatments safely and in an organized manner.
Review, read, note, and initial the electronic health record to document resident information.
Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse.
Administer, assist with, and observe medications using the medication administration record and the Six Rights of Medication Pass.
Ensure medication passes are completed according to times using a mobile medication cart.
Document and initial administered medications, noting refusals or missed doses.
Maintain confidentiality of resident medication information.
Report all resident concerns and medication availability to the RCD or Wellness Nurse.
Restock medication cart after all medication passes.
Assist in checking medication regardless of packaging system.
Assess residents for need of "as needed" medication, document, and report to supervisor.
Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies.
Maintain and clean the Medication Room, carts, and treatment carts for neatness, cleanliness, and availability.
Follow the re-fill process for medications.
Help residents maintain independence and promote dignity and safety according to Sunrise Principles.
Practice standard care precautions, hygiene, and health practices.
Audit medication carts.
Resident Care
Notify RCD of any resident or family concerns.
Attend and actively participate in daily Cross Over meetings facilitated by the LCM.
Conduct Service and Health Updates as directed by RCD.
Participate in the development of Individualized Service Plans (ISP).
Transcribe orders.
Risk Management and General Safety
Partner with community team to ensure compliance with national/provincial regulations and safety policies.
Practice safety procedures at all times, including PPE, fire extinguishers, SDS, and Lockout/Tagout procedures.
Report all accidents/incidents, unsafe conditions, and equipment immediately.
Ensure cords, carts, and equipment are kept out of exits and compliant with fire codes.
Comply with infection control techniques and hazardous material handling.
Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazards immediately.
Ensure oxygen tanks are stored safely and monitored.
Training and Contributing to Team Success
Participate as a team member and commit to team goals.
Demonstrate daily interactions with others that reflect our Team Member Credo.
Serve residents and guests through our Principles of Service.
Engage in overall engagement programs, surveys, and improvement workshops.
Attend regular meetings: Town Hall, Department Team, Cross Over, Medication Technician, and others as directed.
Attend training by RCD and neighborhood coordinators.
May be designated as shift supervisor.
May supervise other Medication Care Managers.
Maintain compliance with required training and regulatory authorities.
Perform other duties as assigned.

Effective written and verbal communication skills.
Demonstrate good judgment, problem solving, and decision-making.
Experience And Qualifications
LPN, LVN, or state/province specific licensed nurse credential.
Maintain certifications as required in states/provinces.
Must maintain CPR and First Aid certification.
Must be at least 18 years of age.
Previous experience working with seniors preferred.
Desire to serve and care for seniors.
All driving team members must sign the driver job description and understand safety regulations.
