The Care Manager/“Designated Care Manager” provides the highest quality care and services to a consistent group of residents and their families in assisted living/long term care and reminiscence neighborhoods. The Care Manager delivers hands-on care and physical and emotional support per each resident's Individualized Service Plan (ISP) while maintaining a safe, home-like environment. The role supports Sunrise Senior Living’s mission to champion quality of life for all seniors and adheres to applicable laws, Sunrise standards, policies, and procedures to promote quality care. Responsibilities & Qualifications

Essential Duties

As part of the Sunrise team, support of our Mission, Principles of Service and Core Values is fundamental. The following responsibilities are representative of the essential functions: Resident Care Review, read, notate, and initial Daily Log to document and learn about pertinent information about residents. Participate in the development of the ISPs and monthly updates. Review designated assignments. Be responsible for a designated group of residents during the shift, know where residents are, and physically check on them throughout the shift. Observe, report, and document symptoms and conditions of residents for changes in condition (skin, behavior, alertness, weight, diet, participation in activities). Attend daily Cross Over meetings with the lead care manager. Notify supervisor and resident care director if a resident has increased care needs. Inform supervisor of any resident changes in condition. Respond to security system and resident call bells promptly; take appropriate action including resetting call bells. Greet guests, family members, residents, and team members; answer, direct and/or respond to phone calls in a timely, courteous, and professional manner. Assist with continence management and dispose of continence products per sanitation standards. Communicate with families and serve as a resource as needed. Help residents maintain independence and physical safety and promote dignity, adhering to Resident Rights and Sunrise Principles of Service. Participate/lead and assist residents with activities of daily living (ADLs) and Invite, Encourage, and Assist (IEA) residents to participate in ISP-listed activities. Engage residents in life skills and life enrichment activities throughout the day in reminiscence. Understand and respond to each resident with empathy, considering communication patterns or history and basic human needs. Maintain an atmosphere of warmth and a calm environment; follow established safety regulations; practice standard care precautions of cleanliness, hygiene, and health. Host and engage in activities with residents daily. Risk Management & General Safety Partner with the community team to ensure compliance with national/provincial regulations, promote risk management programs, and adhere to safety rules and regulations. Practice safety procedures including PPE, fire extinguishers, SDS, and lockout/tagout. Report all accidents/incidents immediately and report unsafe or hazardous conditions/equipment. Keep cords, carts, equipment and other hazards out of walkways and in compliance with fire codes. Understand proper methods for attending to and disposing of bloodborne pathogens, bodily fluids, infectious waste, sharp objects, and hazardous materials; report occupational exposures immediately. Ensure oxygen tanks are stored safely and monitored to prescribed levels. Housekeeping & Laundry Maintain and clean resident rooms, including making beds, trash removal, clothing organization, spot sweeping and mopping as needed. Wash resident laundry as noted in the ISP; wash and fold dining room linens and napkins. Complete assigned housekeeping tasks and maintain common areas clean and tidy. Dining Service Serve meals in the dining room; promote a pleasant dining experience and assist with dining room setup/cleanup as assigned. Participate in pre-meal meetings; follow residents’ diets as indicated on ISP/Confidential Diet Board. Document changes in dining habits; provide room service as needed; practice safe food handling in line with universal precautions. Reminiscence Neighborhood Responsibilities IEA residents in life skills and enrichment activities per ISP and demographic profile; facilitate afternoon social participation; incorporate "Creating Pleasant Days" into daily routines; integrate life skills into daily activities; blend multisensory experiences into the day; participate in monthly letters home with residents and families. Training & Team Contribution Participate as a team member; demonstrate the Team Member Credo; contribute to engagement programs and surveys; attend regular department meetings and trainings; comply with infection control techniques and training requirements; perform other duties as assigned. Core Competencies Ability to stay calm in difficult situations; manage multiple priorities; effective written and verbal communication; strong organizational and time management skills; good judgment, problem solving and decision-making. Experience & Qualifications High School diploma/GED (as required by regulations); CPR/First Aid as required by regulations; must be at least 18 years old; prior experience with seniors preferred; ability to act in residents’ best interests; driving role requires Driver job description and safety duties where applicable.
