The Care Manager/"Designated Care Manager" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home-like environment. The Care Manager/"Designated Care Manager" demonstrates the Sunrise Mission, "to champion quality of life for all seniors" in accordance with laws, Sunrise standards, policies, and procedures. Responsibilities & Qualifications
Responsibilities As a part of the Sunrise team, support our Mission, Principles of Service and Core Values. You will work with residents to promote dignity and independence and respond to their needs as outlined in the ISP. Resident Care Review, read, notate, and initial Daily Log to document and learn about residents. Participate in the development of ISPs and monthly updates. Review designated assignments and be responsible for a designated group of residents during the shift, knowing where residents are and checking on them. Observe, report, and document symptoms and changes in resident condition (skin, behavior, alertness, weight, diet, activity). Attend daily Cross Over meetings with the lead care manager. Notify supervisor and resident care director if a resident has increased care needs; inform supervisor of changes in condition. Respond to security system and resident call bells promptly; take appropriate action including resetting bells. Greet guests, family members, residents, and team members; respond to phone calls professionally. Assist with continence management and dispose of continence products per sanitation procedures. Communicate with families and serve as a resource as needed. Help residents maintain independence and safety while promoting dignity and adhering to Resident Rights and Sunrise Principles of Service. Participate/lead and assist with activities of daily living (ADLs) and encourage resident participation per ISP. Engage residents in life skills and life enrichment activities throughout the day in reminiscence. Understand and respond empathetically to each resident, considering their communication patterns and needs. Maintain a warm, personal, calm environment and follow safety regulations. Practice Standard Care Precautions for cleanliness, hygiene, and health. Host and engage in activities with residents daily. Risk Management & General Safety Partner with the community team to ensure compliance with regulations; promote Risk Management programs and safety rules. Practice safety procedures including PPE, fire safety equipment, SDS, and Lockout Tagout. Report all accidents/incidents and unsafe conditions immediately. Ensure cords, carts, equipment, and hazards do not block exits and comply with fire codes. Understand and practice proper handling and disposal of hazardous materials and exposure to bloodborne pathogens. Report occupational exposures immediately; ensure oxygen tanks are stored safely and monitored. Housekeeping & Laundry Maintain and clean resident rooms; perform laundry and housekeeping tasks as needed. Maintain common areas in a clean and tidy manner. Dining Service Serve meals in the dining room; promote a pleasant dining experience. Assist with dining room setup/cleanup; participate in pre-meal meetings. Follow residents’ diets as indicated on ISP; observe and document changes in dining habits. Respond to dining needs while upholding Sunrise hospitality and service standards; provide room service as needed. Practice safe food handling in accordance with care precautions. Reminiscence Neighborhood Care Manager — Specific Responsibilities IEA residents in life skills and enrichment activities as indicated on ISP and demographic profile. Encourage attendance at afternoon social and integrate life skills into daily routines. Incorporate "Creating Pleasant Days" concepts and multi-sensory experiences into residents’ days. Participate in monthly letters home with residents and families. Training & Team Contribution Participate as a team member; contribute to team goals and engagement initiatives. Demonstrate the Team Member Credo and Sunrise Principles of Service. Attend meetings and training; comply with infection control and regulatory training requirements. Maintain compliance with state/provincial regulations and Sunrise standards; perform other duties as assigned. Core Competencies Ability to stay calm in difficult situations; manage multiple priorities; communicate effectively; strong organizational and time management skills; good judgment and problem-solving. Experience & Qualifications High School diploma/GED (as required by regulations). CPR and First Aid as required by regulations. Must be at least 18 years of age; prior experience with seniors preferred. Desire to serve and care for seniors; ability to act in residents’ best interests. If applicable, drivers of Sunrise vehicles must sign the Driver Job Description and understand safety duties. Benefits & Pre-employment
