You will have the ability to work with physicians that are constantly pushing the bounds of possibility forward, conducting groundbreaking research and providing lifesaving, personalized patient care. This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area.

Essential Job Functions

Plans, monitors and coordinates patient care

Collaborates with the patient, family, care team and multi-disciplinary team to assure appropriate, safe, cost-effective, timely and efficient care

Identifies patient and family needs for consultation with other multidisciplinary specialists and assures needs are met

Assures patient and family educational needs are met

Able to work independently

Collaborates with team (providers, APPs, and nurses) on a daily basis

Attends clinic with physicians performing patient teaching and coordinating procedures

Manages in basket for advanced heart failure patients, refilling prescriptions per protocol, filling out FMLA paperwork, educating patients in heart failure management, and coordinating outpatient procedures and testing

Required Qualifications

Graduate of an NLN accredited School of Nursing

Associate Degree in Nursing

Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire, and make progress toward and complete their BSN degree within 30 months. ADN nurses receive 100% tuition reimbursement toward a BSN degree from an accredited program

Current Illinois RN license

Active BLS certification from the American Heart Association (AHA)

Two years experience in Cardiology

Shift: Days, 7am-5pm; 10-hr shifts

Work Location: Onsite - Hyde Park campus

Compensation & Benefits Overview UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
