Signify Health, part of CVS Health®, is hiring full-time physicians (MD/DO) to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

Responsibilities

Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area (majority of visits scheduled weekdays between 8 a.m. and 7 p.m.).

Primary coverage areas include but are not limited to: Fairfield, New Haven counties.

Provide 40 hours of availability weekly, consistently providing 160+ capacity units per month.

Execute a strong visit completion rate, consistently pacing 110+ completed visits per month.

Execute a collaborating physician agreement, partnering with and overseeing Signify Health-contract nurse practitioners or physician assistants (and fulfilling all state-by-state requirements).

Role Expectations & Requirements

Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s).

Consistently engage with regular manager, team and clinical meetings.

Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics.

Conduct video visits as needed (maintain training, affiliation, and compliance with video visit requirements, regular training required).

Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required).

Maintain all required supplies and associated diagnostic equipment related to DPS offerings.

Maintain active medical licenses (MD/DO) in coverage area(s).

Maintain multi‑state licenses (preferred).

Experience with Signify Health In‑Home Health Evaluations, in‑home visits, or a similar format (preferred).

Be flexible to travel locally and within licensed states; may require hotel stay, car rental, and/or flight up to 20% annually.

Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale), required.

Accommodate field observations and/or “ride‑along” experiences, as requested.

Visit Details During our In‑Home Health Evaluations (and other associated follow‑up visits), you’ll meet people where they are and re‑engage them with their health care during an approximately 45‑minute pre‑scheduled visit.

The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You will be provided a pre‑populated, patient‑specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter people’s current treatment regimen.

Pay Pay range: $145.00 - $145.00 per hour (typical hourly rate).

The amount is dependent on experience, education, geography and other relevant criteria.

This position is eligible for other reimbursement opportunities, including pay per completed Diagnostic and Preventive Service (DPS), as ordered.
