A Day In Life As a Coordinated Entry Specialist As a Coordinated Entry Specialist, you’ll play a crucial role in facilitating and managing the coordinated entry process for individuals seeking housing assistance in the Belknap-Merrimack community of New Hampshire. In this position you will assess client needs, coordinate referrals, and connect individuals experiencing homelessness with appropriate housing resources and support services. The role requires exceptional organizational skills, strong interpersonal abilities, and a deep commitment to helping individuals access stable housing.

Your Day Will Involve

Conduct comprehensive assessments of clients seeking housing assistance, evaluating their housing needs, vulnerability, and eligibility for different housing programs.

Implement and manage the coordinated entry process, ensuring a streamlined and efficient system for assessing, prioritizing, and referring clients.

Collaborate with local community partners, housing providers, and social service agencies to gather and exchange relevant client information and coordinate referrals.

Maintain accurate and up-to-date client records and documentation in accordance with program guidelines and confidentiality protocols.

Provide support and guidance to clients throughout the housing search process, sharing information about available resources, housing options, and eligibility requirements.

Assist clients in completing necessary applications and forms for housing programs, ensuring accuracy and compliance with CAPBM requirements.

Requirements

High School diploma required.

Two years of professional or personal experience with homelessness, mental illness, drug/alcohol abuse, or developmental disabilities.

Strong interpersonal and organizational skills, ability to work independently with minimal supervision.

Knowledge of local human service agencies and CAPBM programs, ability to make beneficial referrals.

Maintain professional boundaries and confidentiality with clients and staff.

Successful completion of non‑violent crisis prevention and intervention training and a criminal record check.

Reliable transportation and a valid driver’s license (some travel may be required).
