Overview
Under the assigned supervisor, the Transition/Aftercare Supervisor is responsible for the direction and management of effective delivery of Clinical Case Management and Aftercare services to youth in a Qualified Residential Treatment Placement (QRTP), Department of Juvenile Justice (DJJ) and Group Home setting (GH). Ensuring usage of evidence-based practices, the assigned QRTP acts in accordance with the provisions outlined in Family First regarding a congregate care facility, the residential programmatic and DCFS contractual guidelines. The Transition/Aftercare Supervisor will ensure supervisees utilize applicable principles of Case Management, Social Work and/or Frameworks which include – Attachment, Regulation and Competency (ARC), Intake/Admission Assessments, Weekly Clinical and Residential staffing, Case/Discharge Planning, Goal Setting, Collaboration with DCFS, Internal and Community Stakeholders. Along with individual mentoring, family linkage, client/family education, behavioral and treatment planning. The Transition/Aftercare Supervisor provides supervision to staff, coaching and clinical supervision to new and existing staff, and supports ongoing development of Case Management and Trauma Informed Skills. They stay up to date on programming, Illinois state guidelines and best practices in Case Management and Clinical Supervision. General Duties And Responsibilities include ensuring compliance with reporting, service quality, programmatic outcomes, admissions processes, and collaboration with internal and external stakeholders; overseeing aftercare for discharged clients; ensuring documentation, funding policies, and contractual obligations are met; and maintaining confidentiality, quality-of-service delivery, and ethical standards. Additionally, the role involves monitoring and reporting, facilitating transitions to next placements, and ensuring appropriate engagement with hearings, reviews, and team meetings. The position requires handling of monitoring, data entry (e.g., Redcap), and adherence to incident reporting and client/ staff grievances processes. The position may require after-hours on-call support. Responsibilities
Reports to the Child Abuse Hotline as directed by the Mandated Reporter Act. Provide exceptional customer service to residential clients, parents, foster caregivers, referring case agencies, and team members. Maintain confidentiality of client/supervisee information and ensure data security. Report exposure to communicable diseases. Submit expense vouchers, renew driver’s licenses, TB/physicals, and automobile insurance on due date. Conduct frequent regional and all-state in-person and virtual team meetings with supervisees, clients, internal and external stakeholders. Participate in agency management meetings and work groups. Complete requests for Case Managers to receive DCFS Email, Redcap, SDS access, RELIAS and/or NCTSN registration. Assist CQI in developing agency documentation to meet contractual requirements and policies. Work to ensure quality-of-service delivery and achievement of programmatic outcome goals per Behavior Management/IPP and contractual obligations (DJJ, QRTP, GH). Manage Admissions Process information for clients matched or referred by DJJ, QRTP, and Group Homes; determine interview status with Clinical Leads. Oversee and support case responsibilities and provide guidance in case planning. Oversee and support client sibling/family visitation and linkage to visiting resources. Participate in and/or attend hearings, Administrative Case Reviews (ACR), staffing, CFTMs, Admission Interviews, and internal meetings. Oversee Aftercare responsibilities for clients discharged for up to 6 months, ensuring ethical service delivery. Review funding source policies and train staff on implementing these procedures via onboarding for Residential and Case Management. Collaborate with office managers, administrative staff, youth care workers, nursing, clinical team, dietary, maintenance, and upper management. Ensure significant incident reporting within the 12-hour reporting period. Participate in collateral and community groups to promote programs and network with the community. Participate in DCFS Contract Monitoring, MCO, and internal/external file reviews. Oversee management of all client Case Management and/or Aftercare digital and on-site records. Facilitate transition to next placement in coordination with care providers and related stakeholders; ensure documentation per DCFS Transition/Aftercare Protocol. Complete monitoring/outcome reports as required. Monitor program expenditures for budgetary/contractual compliance. Adhere to contractual obligations with funding sources; ensure billing accuracy and timeliness via SDS Case Management Notes; meet monthly Case Management billing units. Report critical incidents to DCFS Monitors; manage incident reporting and follow-up. Handle complaints and grievances per Residential and Client/Grievance processes; provide On-Call after-hours support as needed. Qualifications
Education : Master’s in human services related fields, Counseling, Mental Health, and/or Social Work. Experience : 2 to 5 years of progressive responsibility in a Supervisory and/or Case Management, Mental or Behavioral Health role, or equivalent combination of education and experience. Licensure/Certification : LPC, LCPC, LSW, LCSW and Child welfare Licenses preferred, not required. Other: Valid driver’s license and acceptable driving record with OHU required auto insurance coverage. Required Skills
Proficient computer skills in Microsoft Windows; ability to expand software application skills as necessary. Highly organized, flexible, able to manage multiple priorities and meet deadlines. The position requires visual and auditory acuity, manual dexterity to operate office equipment, and the ability to move items up to 50 pounds. Certification in Therapeutic Crisis Interventions (TCI) is required, which may involve restraints and post-event processing.
