WIC Providers Job Summary
The Nurse Practitioner (NP) or Physician Assistant (PA) works in the walk-in/urgent care outpatient setting in collaboration with and as an integral member of a multidisciplinary health care team. The NP/PA manages care for patients presenting with acute illness and injury as well as chronic illness in an outpatient setting. The NP/PA demonstrates an advanced level of medical and nursing knowledge, clinical competence, sound clinical judgment, professional communication, timely and compliant documentation, and is responsible for professional development and competency validation. Full-Time (40 Hours Per Week) Days Rotating Weekends Required Location:
129 N Anderson Lane, Hendersonville, TN 37075 Nurse Practitioner (NP) or Physician Assistant (PA) 1 or more years of prior APP experience required Key Responsibilities
Assessment of health status Performs and documents a complete history and physical examination for acute and chronically ill patients Orders and collects data using appropriate assessment techniques and diagnostic procedures where indicated May serve as first responder for patient status changes and perform required interventions Assesses risks related to care of acute and chronic patients including medications, immobility, nutrition, fluids/electrolytes, immunocompetence, and procedures Coordinates daily rounds with Attending physician teams; presents 24-hour patient report and follows the plan of care Diagnosis: collaborates with interdisciplinary team to diagnose acute and complex chronic conditions Manages diagnostic tests through ordering, interpretation, performance, and supervision Forms differential diagnoses by priority; diagnoses complications and orders appropriate interventions Plans of care: identifies expected outcomes and documents a care plan for complex health needs Uses evidence-based guidelines in a dynamic plan of care applied across the continuum Implements and modifies plan of care; prescribes diagnostic strategies and therapeutic interventions (pharmacologic, non-pharmacologic, and surgical) to achieve outcomes Manages further diagnostic tests through ordering, interpretation, and supervision May perform advanced procedures consistent with privileges, including minor suturing, wound management, line and tube placements/removals Communication and collaboration: maintains ongoing communication with the interdisciplinary team and supports continuity of care across inpatient and outpatient settings Documentation: documents key components of patient progress (progress notes, plans of care, discharge summaries, clinic notes) in the medical record; ensures timely documentation meeting acute care standards Professional practice: demonstrates professional behaviors, mentorship, education of students and staff; engages in research and evidence-based practice; participates in Grand Rounds and related meetings; maintains CEUs and professional memberships Our Nursing Philosophy
