Organization: NFP Provider Team Job Summary: The Advanced Practice Fellow works in collaboration with and is an integral member of an interdisciplinary health care team. The Fellow should have a basic foundation in evidence-based practice and demonstrate medical and nursing knowledge, clinical/technical competence, sound clinical judgment, professional communication skills, timely and compliant documentation. The Fellow may function in various inpatient and outpatient settings throughout the fellowship to optimize learning experiences. Focused learning experiences enable the development of essential specialty-specific clinical and critical thinking skills, promoting top-of-license practice. The environment emphasizes collaborative interdisciplinary care, cost-effective, quality, patient-centered care. A specialty-relevant project will demonstrate evidence-based practice integration and improvement processes. Fellowship duration: 12 Month APP Fellowship Responsibilities

Assessment of Health Status: Perform and document a complete history and physical examination in the medical record. Orders and collects data using appropriate assessment techniques and relevant supporting diagnostic information and procedures when indicated. Assess risks related to care including medication side effects, immobility, immunocompetence, nutrition, fluids/electrolytes, invasive interventions, and diagnostics. Participate in daily huddles or rounds with the interdisciplinary team and collaborate on the ongoing plan of care across the continuum. Diagnosis: Collaborate with the interdisciplinary health care team to diagnose acute and complex chronic conditions. Manage diagnostic tests through ordering, interpretation, performance, and supervision. Formulate differential diagnoses by priority. Diagnose complications and identify appropriate interventions. Plan of Care: Identify expected outcomes from diagnosis and document a care plan addressing acute, complex, and chronic health care needs. Utilize evidence-based practice guidelines or protocols in an individualized, dynamic plan of care across the care continuum. Implementation: Prescribe diagnostic strategies and therapeutic interventions (pharmacologic, non-pharmacologic, and, as needed, surgical) to achieve expected outcomes. Manage additional diagnostic tests through ordering, performance, and interpretation. May perform advanced procedures consistent with privileges and competency validation. Communication and Collaboration: Maintain ongoing communication with the interdisciplinary team and facilitate communication with patients, families, and staff to promote continuity of care across inpatient and outpatient settings. Documentation: Document key components of patient progress via daily progress notes, transfers, discharge summaries, and/or clinic notes. Ensure timely documentation that meets compliance standards while accurately capturing acuity and complexity. Professional Practice: Provide preceptorship, mentoring, education and instruction of students and nursing staff as needed. Seek opportunities for engagement in research and evidence-based practice. Attend Grand Rounds, AP Council meetings, faculty and unit meetings, MM & I presentations, and other educational opportunities. Maintain continuing education to sustain licensure and certification; participate in required professional development fellowship curriculum. Qualifications
