Case Manager 2 Multnomah County – Department of County Human Services (DCHS) Regular Represented – Non‑Exempt position

Pay Range $31.03 – $38.00 hourly

Overview This role provides case management services for children and adults with developmental disabilities. The Case Manager 2 will coordinate and ensure delivery of medical, social, educational, and other supports required by individuals on the state DDSD waiver program, working closely with state, community, and provider partners.

Responsibilities

Manage a caseload of clients eligible for DDSD case management services.

Monitor services through home visits, provider visits, and phone calls to ensure appropriate financial, medical, training, educational, and transportation support.

Develop and complete Individual Service Plans during staff and special meetings at client families’ homes and provider settings.

Coordinate resources to meet consumer and family service needs.

Prepare and submit routine and special reports.

Assess and update the level of functioning and support needed via TXIX Waiver Forms, submitting for state approval and annual updates.

Inform the state when a client leaves a funded facility.

Maintain case records and documentation per federal, state, and local policy; modify necessary documentation to implement program changes.

Advise consumers/families on placement, care plans, financial planning, and interpersonal dynamics.

Liaise with educational systems to support children’s access to services that meet developmental disability needs.

Report possible protective services issues and provide preliminary information for investigations.

Use Tableau data to analyze overdue tasks and manage priority service areas.

Act as liaison among residential providers, licensing specialists, service coordinators, supervisors, ACHP, and IDDSD management on health, safety, and regulation issues.

Qualifications

Understand and respect diverse cultures; provide courteous client service.

Organize and prioritize work to meet deadlines independently or as part of a team.

Apply federal, state, and local laws, regulations, and policies related to program operations.

Research and analyze information, make recommendations, and present findings.

Knowledge of case management methods, assessment, interviewing, risk intervention, and resource utilization.

Familiarity with public assistance, Medicaid, protective services, referral centers, and crisis resources.

Conflict resolution, de‑escalation skills, and ability to facilitate team meetings for consensus.

Minimum Qualifications / Transferable Skills

Bachelor’s degree in social work, psychology, sociology, criminal justice, or related field (or equivalent experience).

Three (3) years of progressively responsible research, field, investigative, or case management experience in social services, court, corrections, or protective services.

Valid driver’s license.

Pass a criminal background check.

Preferred Qualifications / Transferable Skills

Demonstrated ability to use transferable skills developed through education, work, military, or life experience.
