Overview
Clinical Social Worker-LCSW-24 Hours Days/Weekends at Boston Medical Center (BMC). Position Summary: Utilizing clinical social work techniques and theory, consults on, assesses, and intervenes with high-risk patients and their families regarding the impact/amelioration of emotional and social problems. Duties include conducting psychosocial assessments, making a psychosocial diagnosis, planning and implementing effective treatments, evaluation, education, supervision, coaching and mentoring of the patient (client) and family members. The position consults with, collaborates and communicates with a wide range of social, governmental and legal agencies, courts, schools, clinics, other hospitals, physicians and other sources in the overall care of the patient. Assesses patient and family to ascertain emotional and social problems. Utilizes the department’s psychosocial assessment tool as a guideline. Identifies issues to be addressed during the patient’s care and formulates a plan. Reports suspected cases of abuse and neglect and follows up to ensure timely consultations and compliance with legal mandates. Identifies need to involve patient’s family through the initial assessment process and makes contact on the same day if indicated. Consults and collaborates with healthcare team members to ensure patient’s emotional and social needs are addressed. Provides psychotherapeutic treatment and/or psychosocial support to patient and family to reduce identified problems. Ensures inpatient and family remain apprised, involved, supported and treated through prescribed clinical relationships with at least weekly contacts and additional contacts as care plans change. Uses social work techniques to help patient and family cope with illness or social problems to improve functioning and emotional state. Begins inpatient assessment within 24 hours of referral and completes within two interviews or seven working days; begins outpatient assessment as soon as permissible and completes within two interviews; completes ED patient assessment within four hours of discharge. Maintains patient confidentiality according to hospital policy and uses professional discretion in documentation. Documents each patient encounter in the medical/department records with date, time, title and signature. Provides clinical supervision to department personnel as assigned. Complies with departmental, regulatory and professional documentation and data collection requirements. Submits department statistics and other records within required time frames. Maintains competency in areas of responsibility to ensure quality care. Attends scheduled staff meetings and in-service education with punctuality and consistency. Obtains continuing education units necessary to maintain licensure. Supports department operations by participating in programs and adhering to policies and procedures. Supervises MSW and BSW students as assigned. Identifies operational weaknesses and recommends improvements. Participates in department committees and represents the department within the hospital and the community. Maintains a neat work area and equipment; reports any malfunctions; adheres to emergency procedures. Conforms to hospital standards of conduct, including patient rights, to provide high-quality customer service and patient care. Notifies supervisor when patient populations outside regular service are present and seeks appropriate guidance for specialized needs. Education & Requirements
Education : Requires Master’s Degree from an accredited school of Social Work. Certificates, Licenses, Registrations : Requires Massachusetts Licensed Certified Social Worker (or obtained within six months of employment). Experience : Generally requires one to two years on-the-job training/experience related to social work processes and community resources in health, illness, and disability. Knowledge and Skills
Knowledge of community resources and agencies serving children and families. Advanced interpersonal skills to interact effectively with healthcare professionals, develop therapeutic relationships with patients and families, and supervise graduate students. Ability to work in a fast-paced, dynamic environment; sense of humor valued. Ability to work in an interdisciplinary team.
