Overview
PERMANENCY SPECIALIST (UMP) - GALESBURG The State of Illinois is committed to a diverse workforce. Bilingual skills are welcome. This position is within the Upward Mobility Program (UMP) and located in Galesburg, IL.
Responsibilities
Provide professional child welfare casework services to children and families under general supervision.
Determine needs and coordinate placement of children in substitute care facilities.
Conduct interviews with clients and professionals to obtain information for diagnosing problems and developing treatment plans.
Transport clients as needed for protective custody, placement changes, and other duties related to care.
Prepare and maintain case documentation for planning, implementation, and evaluation of services.
Participate in case staffings and multi-disciplinary conferences to assess required services.
Perform other duties within the scope of the above responsibilities as assigned.
Minimum Qualifications
Preferably a master’s degree in social work; OR a master’s degree in a related human service field with one year of directly related professional casework/case management experience; OR a bachelor’s degree in social work with one year of directly related professional casework/case management experience; OR a bachelor’s degree in a related human service field with two years of directly related professional casework/case management experience.
The degree program in social work must be approved by the Council on Social Work Education; the degree must be from an accredited college or university.
Experience must relate to family preservation, family reunification, adoption, youth development, counseling, advocacy services, or a related field as described.
Conditions of Employment
Requires a valid driver’s license, daily access to an automobile, and proof of vehicle insurance.
Requires ability to pass a background check.
Requires physical, visual, and auditory ability to perform assigned duties.
Overtime may be required, including evenings and weekends, and being on call in a 24-hour capacity.
