Position: Medication Nurse, LVN / LPN (Full Time)
Location: Ivy Park at San Jose
Shifts: Tuesday - Saturday (7:30am-4:00pm)
Pay: $31.00 - $34.00 per hour

Responsibilities Perform nursing duties as directed and in accordance with the Nurse Practice Act, including administering medications and performing treatments as ordered by a physician. Administer injections, suppositories, enemas, and wound‑care medications. Coordinate medication services with residents, pharmacy, and physician. Order and track all medication changes, refills, and deliveries with pharmacies. Prepare medication for distribution to residents per company policy. Assist residents with taking their prescribed medications. Observe and verify medication ingestion or application, documenting refusals and notifying supervisor. Maintain accurate resident medication and care records per company policy. Keep medicine rooms and carts orderly, clean, and appropriately stocked. Control medication room/cart access at all times. Monitor psychotropic medication use to align with physician orders and resident behavior. Participate in narcotic counts at shift start and end, reporting discrepancies to supervisor. Communicate with physicians, family members, and vendors as needed. Understand common medical abbreviations, symbols, and terms used in medication administration.
Qualifications Licensed Vocational Nurse or Registered Nurse with a valid California nursing license in good standing. Prefer one year of experience assisting with medications at a Residential Care Facility for the Elderly. Knowledge of assisted living and dementia care practices. Ability to work with seniors and interact with cognitively impaired individuals. Flexibility, adaptability, and decision‑making in stressful situations with prioritization of tasks. Strong information processing and follow‑through on written or oral instructions. Problem‑solving skills and moderate math ability. Pass a Criminal Background check and Health Screening tests, including physical and TB tests. Compliance with vaccination, daily screening, surveillance testing, and PPE requirements as per Oakmont Management Group policy.
