Description This is a full-time position. Monday-Friday, 8:00AM-4:30PM.
Program Coordinator (Community Employment) This Program Coordinator is directly responsible to the Program Coordinator Team Leader. Also, this Program Coordinator will be closely involved with the Quality Assurance and Safety Specialist, including taking directions pertaining to Community Employment related tasks.
This position is a full-time, professional-level role that will include travel to various locations served by Nishna.
Specific Job Duties and Responsibilities
Assume responsibility for persons served during their course of rehabilitation; become familiar with their backgrounds.
Evaluate individual needs and recommend appropriate services.
Develop and write Person-Centered Plans with Program Coordinators, Nishna staff, family members, guardians, school personnel, and referring agencies.
Complete quarterly Progress Reports and all other reports related to the Person-Centered Plan.
Work with supervisors in various service areas to implement appropriate services and plans.
Document individual performance, including activities and behaviors; keep charting, General Event Reports, and daily reports up-to-date.
Attend meetings (facility, intake, annual/plan) to gather input for establishing training objectives.
Maintain communication regarding individual information with outside agencies (social workers, case managers, residential providers, guardians).
Provide counseling as needed.
Coordinate meetings involving the individual and Inter-Disciplinary Team Members.
Distribute all Person-Centered Plans and progress information forms to staff with adequate time for completion; follow up as needed.
Assist the individual in completing required documentation and forms such as consents and Service Agreements.
Observe and evaluate individuals on a quarterly basis.
Ensure goals are entered into the electronic documentation system before new plans or addenda take effect.
Assist with referrals, discharges, and follow-up of individuals who have completed the program or been terminated for other reasons.
Review individual performance to ascertain program design appropriateness and data collection methods.
Keep critical individual information for case records updated.
Apply behavior management and crisis intervention strategies to ensure safety during behavioral crises.
Engage in hands‑on training in residential, community employment, and day habilitation services.
Follow up on General Event Reports as needed.
Participate in the agency’s safety program, including emergency drills and CPR/first‑aid training.
Work with physically and verbally aggressive individuals.
Cooperate with agency investigations.
Ensure proper funding is obtained and maintained.
Participate in monthly Member Services, Program Coordinator, and Community Employment meetings.
Assist with case record review and retention as directed.
Handle duties assigned by the Program Coordinator Team Leader, Quality Assurance and Safety Specialist, Director of Member Services, or Executive Director.
Maintain CARF accreditation by maintaining records and documentation per accreditation standards.
Complete community employment surveys and file audit corrections assigned by the Quality Assurance and Safety Specialist.
Requirements Qualifications/Requirements
Prefer bachelor’s degree in human services, education, or related field, or equivalent experience in human services.
At least one year experience in a direct care or educational position.
Participation in continuing education related to job duties.
Ability to understand and relate to individuals with disabilities.
Professional and personal image and attitude to represent the agency.
Prefer experience with Medicaid‑funded services.
Experience with computer use; proficient in Excel, Word, Outlook, and electronic documentation systems.
Above‑average verbal and written communication skills.
Good judgment, confidentiality, and self‑motivation.
Adaptability to schedule changes with little notice.
Interpretation skills for various assessment tools.
Ability to initiate and organize plans for achievement.
Strong human‑relation skills for effective public interaction.
Compassion for people with disabilities.
Valid Iowa chauffeur license.
Availability during business hours Monday‑Friday; flexible scheduling.
Teamwork orientation.
Work under pressure to meet deadlines.
Minimum 40 hours per week.
Must not have a serious infectious or communicable disease; legal convictions related to abuse or assault prohibited.
Physical Requirements
Travel between agency locations.
Carry necessary paperwork and supplies in a company‑provided locked bag, ensuring HIPAA compliance.
Assist individuals with mobility and balance.
Provide physical assistance to maintain safety.
Lift up to 55 pounds.
