Short Description CODER II – OUTPATIENT demonstrates proficiency in coding multiple single‑visit outpatient/same‑day surgery accounts, including Observation, Hematology/Oncology, Gynecology/Oncology, Urology, Orthopaedics, General Surgery, Gastroenterology, Obstetrics, Gynecology, Podiatry, Ophthalmology, Dental, ENT, Pain Management, Neurology, Emergency, and Diagnostic services. The role supports Revenue Cycle goals for timely billing and ensures accurate use of ICD‑10‑CM, PCS, HCPCS, CPT, and other coding references. The position addresses NCCI, OCE, LCD, and other coding edits, maintains 95% accuracy in diagnoses and procedures, and performs charge reconciliation to guarantee compliance and reimbursement. Regularly reviews coding literature, stays current on coding guidelines, and pursues continuing education.
Responsibilities
Utilize ICD-10-CM, PCS, HCPCS, and CPT to accurately code outpatient visits and same‑day surgeries.
Address NCCI, OCE, LCD, and other coding edits as applicable.
Examine complete medical records to determine and sequence principal and secondary diagnoses, procedures, and complications.
Assign appropriate ICD-10 and CPT codes with 95% accuracy.
Perform charge reconciliation to ensure all submitted charges are posted accurately for compliance and reimbursement.
Maintain consistent performance and steady productivity.
Hold accounts when additional information is required and contact CDI/leadership/medical staff as needed.
Regularly review coding literature, remain current on new or revised coding guidelines, and share information with colleagues.
Complete all assignments as directed by management reliably and conscientiously.
Pursue continuing education both inside and outside the hospital.
Work as a team member to meet department goals.
Perform all related duties or special projects as assigned.

Experience Required 0–2 years of Health Information Management, Coding, or Billing experience. Must demonstrate proficiency in coding multiple outpatient services, including Observation, Multi‑specialty Oncology, Same‑Day Surgery, Endoscopy, Emergency Department, etc. Knowledge of NCCI, OCE, and LCDs mandatory.
Education Requirements High School Diploma required.
License/Certification Requirements One or more of the following credentials: RHIA, RHIT, CCS, CIC, COC, CPC, CCA, CCC, CIRCC, CCVTC, or any core or specialty credential of AAPC or AHIMA.
