Apply Keck Medicine of USC Hospital Alhambra, California In accordance with federal coding compliance regulations and guidelines, use current ICD-10-CM, CPT-4, and HCPCS code sets/systems to accurately abstract, code, and electronically record into the 3M Coding & Reimburse System (3M-CRS) & the coding abstracting system (3M-ClinTrac), all diagnoses and procedures documented by physicians in outpatient medical records. Address OCE/NCCI edits within 3M-CRS and those returned from the Business Office. Understand PFS coding/billing processes & systems such as PBAR and nThrive/MedAssets/XClaim to ensure timely claims with appropriate codes. Perform other coding department duties as assigned by HIM management. Essential Duties Outpatient Ancillary/Clinic Visit/Emergency Department coding using ICD-10-CM, ICD-10-PCS, CPT/HCPCS, and Modifier classifications; abstract patient information as required by coding laws and guidelines. Review entire medical record; accurately classify and sequence diagnoses and procedures; capture all documented conditions affecting current care or treatment, including medical necessity diagnoses, co-morbidities, historical conditions, or external causes of morbidity. Enter patient information into inpatient/outpatient medical record databases (ClinTrac/HDM); ensure accuracy and integrity of UB-04 & OSHPD data prior to billing interface and claims submission. Collaborate with HIM Coding Support and/or CDI Specialist to obtain documentation for complete medical records and optimal code assignment. Assist in correcting regulatory reports (e.g., OSHPD) as requested. Maintain attendance, punctuality, professionalism, and consistent task completion; adhere to HIM policies and procedures. Achieve minimum coding accuracy of 95% as measured by internal/external reviews; achieve 95% abstracting accuracy for UB-04 and OSHPD data elements. Assist in ensuring medical records contain information necessary for coding/abstracting; identify education needs and pursue self-improvement. Act as a resource on coding questions; improve MS-DRG, APR-DRG, SOI, ROM, and APC/HCC assignments per coding guidelines. Maintain productivity standards; drop 95% of patient bills within 5 days after discharge and complete remaining within 2 weeks. Assist other coders and departments with coding issues and interim bill generation; monitor unbilled accounts to prioritize oldest records. Maintain AHIMA/AAPC credential(s) as specified; attend coding/CDI seminars and CEUs; stay current with ICD-10 guidelines and coding literature. Participate in daily huddles and quality improvement activities; communicate process changes to leadership as needed. Work effectively with management, clinical staff, and external customers; ensure timely follow-up on information requests. Demonstrate proficiency with EHR systems (Cerner/PowerChart, Coding mPage), 3M-CRS Encoder, ClinTrac, PBAR, HDM/HRM/ARMS Core, and 3M 360 Encompass/CAC. Perform other duties as assigned. Required Qualifications High school or equivalent; specialized/technical training in Medical Terminology, Anatomy & Physiology; certified coding course. Pass hospital coding test (score ≥70) or waiver for former USC coding staff with prior audit standards met. Experience using computerized coding/abstracting databases and encoding/codefinder systems. Preferred Qualifications Experience in ICD-9/ICD-10 and CPT/HCPCS coding of Outpatient Ancillary/ED medical records in hospital or outpatient clinic settings. Required Licenses/Certifications Certified Coding Specialist (CCS), CCS-P, CPC, or COC; or RHIT/RHIA as alternative certifications with required examinations within six months if not holding a national coding certificate. Fire Life Safety Training (LA City) or obtain within 30 days of hire if applicable. Hourly rate range: $33.00 - $54.02.
