Base pay range
$104,000.00/yr - $143,000.00/yr
Responsibilities
Assess and evaluate members’ conditions, with a focus on medication therapy, through telephonic interactions to ensure optimal wellness.
Develop and implement comprehensive care plans; monitor patient progress through regular assessments.
Utilize advanced telecommunication systems to provide medication education.
Work independently while adhering to established protocols and guidelines.
Make a meaningful impact on the lives of patients with serious and rare health conditions within a supportive, professional environment.
Participate in a Specialty Clinical Team with tasks based on business needs.
Handle daily inbound and outbound calls to discuss medication therapy, side effects management, and adverse event reporting when appropriate.
Other tasks not directly phone-call related as assigned, including but not limited to:
Pharmacist Care Plan/Treatment Review
Review prescriptions for accuracy
Check for drug-drug interactions
Evaluate disease state appropriateness, dosage, directions, and therapy
Utilize electronic resources and guidelines to support professional decision making
Qualifications Required Qualifications
Pharmacy degree from an accredited college of pharmacy (Bachelor of Science or PharmD)
Active pharmacist license from Florida, Texas, Ohio or Arizona Board of Pharmacy
Candidate must reside and work within the state of Florida, Texas, Ohio or Arizona
3+ years of experience as a pharmacist in a high-volume retail, hospital or mail order setting
Ability to participate in Federal prescription programs
Ability to work independently and within a team to achieve goals and deadlines
Strong problem-solving skills and ability to utilize resources for professional decision-making
Focused, accurate, and productive work ethic
Excellent verbal and written communication skills for team and patient/caregiver interactions
Proficiency with computers and ability to troubleshoot technology issues
Knowledge of Microsoft Office applications (Word, Excel, Outlook, OneNote)
Preferred Qualifications
Pharmacist Board Certification – Oncology, Specialty, Geriatric, Pharmacotherapy, Ambulatory
Proficient in Spanish (if applicable, specify on application/resume)
Previous experience in a Specialty Pharmacy setting
Experience with Pharmacy Benefits Management (PBM)
Experience with Commercial, Medicare and/or Medicaid
Clinical experience in managed care
Additional Information
This is a remote position; candidates must reside in Ohio, Florida, Texas or Arizona
Weekly schedule: Monday–Friday, 10:00 a.m.–6:30 p.m. EST
2 closing shifts per week: 11:30 a.m.–8:00 p.m. EST
Holiday and Saturday shifts (9:00 a.m.–5:30 p.m. EST) rotate based on business needs
Overtime may be required due to business needs
Work-from-home requirements include a suitable home office setup and secure handling of PHI/HIPAA information
Work Style: Remote
Travel: Occasional travel to Humana offices for training or meetings may be required.
