Job Summary Coordinates, facilitates, and performs interdisciplinary plan of care through assessment and evaluation of patient condition and effectiveness of care under the direction of the attending physician by traveling to patient homes, community‑based residential facilities, assisted living locations, hospitals, or other settings to meet patient and family/caregivers, assess patient care needs, and acutely manage care in accordance with policies, procedures, and regulations of home care and/or hospice services.
Primary Responsibilities
Assesses and coordinates patient discharge planning needs with members of the healthcare team.
Performs point‑of‑care testing in accordance with policies and procedures.
Collaborates, consults, and communicates with physician regarding the plan of care and changes to patient status; provides updates and recommends plan revisions; documents status and communication in adherence with regulations.
Coordinates and communicates with multidisciplinary team members and outside agencies to achieve expected patient outcomes and education through end of life or discharge; ensures accurate and complete documentation and coordination of care plans.
Completes admission paperwork and verifies insurance information; completes comprehensive medical assessment and enters data into the clinical computer program.
Provides skilled nursing care within the established plan of care, including vitals, wound care, IV care, ostomy care, blood draws, injections, and physical assessment.
Documents visit notes, treatments, and care; follows visit pattern according to physician orders and ensures required visits are completed timely and accurately.
Collaborates with patient, patient’s family, and healthcare team to develop comprehensive care plans based on unique needs; responds to calls and completes follow‑up to address needs, issues, or concerns.
Maintains a constant state of alertness and works safely.
Performs other duties as assigned.
Qualifications
Graduate of an accredited school of nursing or equivalent education for licensing.
No prior experience required.
Proof of motor vehicle liability insurance that meets minimum state requirements upon hire and upon request.
State licensure: Registered Nurse (RN) licensed in Illinois; or applicable compact RN license in Missouri, Oklahoma, or Wisconsin.
Basic Life Support HealthCare Provider (BLS HCP) certification from the American Heart Association.
Valid driver license and driving record (MVR) acceptable.
Physical requirements for standing, walking, lifting up to 25 lbs, use of speech and vision, frequent driving, and data entry.
Working Conditions
PRN / Per Diem shift with variable weekly hours (0 - variable).
Employee status; scheduled weekly hours may vary.
