Position Summary:
Under the supervision of the Admin Nurse III, the Dietitian Nutritionist will provide services related to medical nutrition therapy in the treatment and prevention of disease.
Duties & Responsibilities:
Assesses nutritional, health and dietary needs of patients.
Develops meal plans for patients and evaluates the effects of meal plans, then revises the plans as needed.
Counsels patients on nutrition issues and healthy eating habits.
Documents patient dietary and nutritional assessments, and patient progress within the EMR (Electronic Medical Record).
Maintains regular nutrition counseling sessions with patients in person or virtually to offer patients the opportunity to voice concerns and have ability to provide objective feedback.
Provides ongoing communication between patient and the family to assure active engagement and achievement of nutrition goals.
Attends division meetings with clinic support staff and providers to ensure superior nutritional and dietary care is provided to patients and families.
Assists in developing and revising patient education materials.
Possesses an up to date knowledge of new concepts and advancements in dietetics and nutritional science research.
Consults with physicians and healthcare personnel to determine nutritional needs and diet restrictions of patient.
Participates in the training and education to medical students, Residents, RD students, and new hire RDs.
Develops, evaluates, and updates nutrition care policies and procedures to achieve cost containment and improved patient outcomes.
Participates in research projects as assigned.
Performs other related duties and participates in special projects as assigned.
Minimum Qualifications:
Possession of current Registered Dietitian licensure from the Illinois Department of Financial and Professional Regulations and credentials and registration from the Commission on Dietetic Registration (CDR) for the Academy of Nutrition and Dietetics.
