What You’ll Do
Get the Best Training: Start with a two-week clinical orientation to learn essential procedures, our EMR system, safety protocols, and compassionate care practices.
Be the Expert: Assess patients with complex health needs, develop personalized care plans, and ensure optimal outcomes.
Make a Connection: Educate and guide patients and families through their healthcare journey, helping them understand conditions and the path to better health.
Collaborate for Success: Work with a team of VNA clinicians and external providers to ensure seamless care and optimal results for every patient.
Document with Purpose: Record and track changes in patient conditions, updating care plans to reflect progress and needs.
Qualifications
Registered Nurse (RN) with a bachelor’s degree in nursing (preferred).
1+ year of clinical experience as an RN (homecare experience is a plus).
Strong verbal and written communication skills to guide and educate patients and families.
Proficient with technology, including EMR systems and digital tools to enhance patient care.
A reliable car and valid driver’s license for travel to patient homes.
Physical Demands/Working Conditions
Flexibility to work in various environments and use clinical equipment.
Comfortable using a phone, computer, and other digital tools to stay connected with the team and patients.
Direct patient/family contact and presence as part of care delivery.
Compensation and Benefits In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000 to USD $110,000. The offer considers role scope and responsibilities, candidate experience and education, skills, internal pay equity, and market conditions.
