Overview We are seeking
Case Manager II
to work on our
Waiver Case Management
team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. We value work-life balance and have manageable caseloads that are on average 30.
Hours:
Full-Time, Monday–Friday, business hours 8am-4pm
Locations:
Dakota County | Minnesota | Hybrid
Hiring Range:
$24.00 - $25.35/Hourly based on qualifications
Hiring Bonus:
$1000 to external candidates

Responsibilities
Attending bi-annual meetings with clients to monitor services
Develop and maintain a collaborative relationship with clients, serving as a role model for positive, proactive, and respectful communications
Assessing client needs and ensuring continuity of care, including outreach and consultation with other partners and providers
Maintain regular contact with collaborative partners and make referrals to professionals, schools and community agencies
Making referrals and authorizing services based on needs
Maintaining records and documenting all coordination activities
Documenting all interventions, interactions and relevant information to the treatment plan in alignment with professional and organizational standards
On-site presence in the Twin Cities, Minnesota
Requirements
Valid driver’s license, excellent driving record and current auto insurance
Education and Experience
For Hennepin and Dakota County Waiver Case Manager II positions: Bachelor’s Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field; OR Bachelor’s Degree in any field plus one year of experience as a social worker/case manager/care coordinator in a public or private social service agency
For Anoka and Ramsey County Waiver Case Manager II positions: Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or related field, AND one year of experience in the related field
