Makes daily rounds on unit to observe, examine, interview residents, evaluate staffing needs, monitor regulatory compliance, inspect care environment, and evaluate staff interactions and clinical skill competency. Audits 24-hour report as completed by Unit Charge Nurses and reviews clinical records to identify problems, ensure timely responses to residents’ needs, evaluate documentation skills of unit nursing staff, and ensure regulatory compliance with documentation requirements. Listens to verbal shift report, following up on significant changes in condition, admissions, transfers, discharges, initiation or use of restraints, medication safety, incidents, and resident/family concerns. Analyzes data from rounds and record reviews, resolves problems, and participates in quality assurance activities as directed by the Quality Assurance committee. As the nurse responsible for residents on the unit, conducts or coordinates the resident assessment process including accurate completion of the State-approved Resident Assessment Instrument. Participates as a member of the interdisciplinary team in development of the plan of care for each resident, with input from the resident and/or family, including measurable objectives and timetables and accommodating resident needs and preferences. Attends interdisciplinary conferences to plan care and provides opportunities for direct care staff to participate. Supervises nursing staff in the implementation of plans of care to help residents attain or maintain highest practicable well-being. Maintains ongoing communication with other disciplines to evaluate resident responses to interventions and revise plans of care as needed. Informs residents about care and treatment and any plan changes; notifies physicians and designated family members when there are significant changes or incidents. Interacts with residents, families, and legal representatives to resolve questions and concerns; resolves grievances with appropriate escalation. Educates and develops unit nursing staff by coaching on policies and procedures and identifying learning needs. Monitors infection control practices among unit staff. Assists Unit Charge Nurse with staffing assignments, adjusts patterns based on census and acuity, and communicates staffing challenges promptly. Performs other duties as assigned. Qualifications
Must be licensed in the State of Wisconsin as a Registered Nurse. Previous supervisory experience in a long-term care and/or clinical setting. Must have current CPR certification.
