Senior Case Manager – North Star Initiative
North Star Initiative is dedicated to restoring women from trafficking and empowering survivors to reclaim their lives through comprehensive, trauma-informed care. The Senior Case Manager provides individualized case management to residential residents, applying trauma-informed principles to support their goals within the program framework. Key Responsibilities
Conduct needs assessments and develop Individualized Service Plans (ISP) for each resident. Coordinate resident treatment plans with the clinical care team under the direction of the Clinical Director. Support resident goals through coordination of job searches, medical care, legal services, budgeting, appointments, and other resources. Collaborate with Programming Manager and Residence Manager to schedule and minimize conflicts. Track progress and complete outcome surveys, such as Assessment of Survivor Outcomes (ASO), reporting results to the clinical care team. Meet weekly with individual residents for case management sessions. Maintain accurate, confidential electronic and paper resident files according to organizational policies. Act as liaison between residents and community resources, processing referrals, interviews, and new resident onboarding. Assist with medication management as part of the medication team. Network with community agencies to cultivate resources that may benefit resident goals. Ensure compliance with all mandated legal and regulatory requirements. Attend meetings, trainings, and abide by North Star Initiative policies and procedures. Be available for on‑call staff and resident support as needed (typically one week/month). Assist in the development of trauma‑informed policies and protocols. Provide updates on residents’ progress to direct care staff. Support and oversee case management within the Berks‑based programming. Perform additional duties as determined by program needs. Required Qualifications
Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint) and database systems. Commitment to Christian faith; purpose and intentional relationship with Jesus Christ. Understanding of sex trafficking, exploitation, addiction, and trauma-informed care. Cultural competency and the ability to communicate effectively with individuals of all backgrounds. Ability to remain calm in stressful situations and support residents in pre‑crisis or crisis situations. Self‑starter attitude with a strong drive to follow up on assignments. Willingness to sign the North Star Initiative Statement of Faith (Code of Beliefs) and Kingdom Living policy. Current and active membership or regular attendance at a local church. Willingness to fully support North Star Initiative’s core values and ministry philosophy. Preferred Qualifications
Master’s Degree in Social Work (MSW). Experience working with survivors of human trafficking and trauma. Four years’ experience in direct care with knowledge of social work ethics. Education & Experience
Minimum: Bachelor’s Degree in Social Work (BSW). Minimum experience: Two years working in human services. Preferred: MSW and four years of direct-care experience. Physical Demands
Frequent standing, walking, and use of hands; occasional lifting up to 25 pounds. Requires close vision and the ability to adjust focus. Travel and driving required. The work environment is typically moderate in noise level. Employment Type & Details
