Work Schedule Part‑time, 20 hour a week position with flexible scheduling.
Overview Optum is seeking a Part‑time Nurse Practitioner to join our HouseCalls team in Callaway and Osage County, MO. As part of the Home and Community Care team you will provide annual clinical assessments to patients in the comfort of their homes, helping to identify and reduce health risks and coordinate follow‑up care to improve health and well‑being.
Responsibilities
Conduct in‑home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs and comprehensive physical exam.
Perform evidence‑based practice screenings, including point‑of‑care testing as appropriate.
Identify diagnoses for care management and active medical management.
Formulate a list of current and past medical conditions using clinical knowledge and judgment.
Communicate findings to member's PCP to address potential gaps in care.
Identify urgent and emergent situations and intervene appropriately.
Educate members on disease process, medication, and compliance.
Qualifications
Active NP license in Oklahoma or ability to obtain by start date. Licensure must be unencumbered.
Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult‑Gerontology Primary Care, or Adult‑Gerontology Acute Care, or ability to obtain by start date.
Ability to complete physical requirements of the job including lifting a 30‑pound item, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform physical assessment.
Reliable transportation to complete home visit assessments.
Preferred Qualifications
1+ years of clinical experience at the highest level of education or clinical setting.
Experience in gerontology, cardiology, internal medicine, or endocrinology.
Home health care or home visit experience.
Compensation and Benefits Compensation generally ranges from $104,500 to $156,000. Total cash compensation includes base pay and bonus and may increase over time based on productivity and performance. Benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (subject to eligibility requirements).
