Transplant Nurse Practitioner

Salary:
$139,776.00 – $211,432.00 per year (base pay)
Pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on factors such as seniority, education, training, relevant experience, certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
Job Summary

The Transplant Nurse Practitioner is an important member of the multidisciplinary transplant team. Responsibilities include overseeing care for organ transplant candidates and potential donors, collecting clinical information, summarizing medical data, presenting information to the transplant team, arranging diagnostic testing and treatment procedures, providing patient and family education, interfacing with multiple providers to ensure optimal pre‑post‑transplant care, and participating in 24‑hour on‑call coverage of patient‑related phone calls.
Essential Responsibilities
Perform history and physical examinations, formulate care plans, order and interpret laboratory and diagnostic tests, and educate patients and families.
Act as an educator and clinical resource through consultation, formal and informal education, literature review, and participation in seminars and conferences.
Coordinate patient care to provide safe, effective, efficient, timely patient‑centered care; liaison between nurses, clinical staff, and consult services.
Consult with physicians or surgeons regarding complex diagnoses.
Document clinical encounters clearly, precisely, and in a timely manner.
Collaborate with other health professionals to determine patient and family health needs; demonstrate interdisciplinary collaboration.
Provide direct patient care and management for all opioid patients.
Required Qualifications
Master’s degree in Nursing (Master’s preferred).
Nursing license; Basic Life Support (BLS) certificate.
0–1 years related work experience.
MA Board authorization to practice as APRN.
Valid prescriptive authority from both MA DPH‑DCP and Federal DEA.
Health system computer skills (web‑based applications, Microsoft Office Suite).
Preferred Qualifications
Experience in a teaching hospital and specialty area expertise.
Experience in pain management.
Competencies Written Communication:
Ability to summarize and communicate moderate‑complex information in varied written formats to internal and external customers.
Oral Communication:
Ability to comprehend and communicate complex verbal information in English to staff, patients, families, and external customers.
Knowledge:
In‑depth knowledge of concepts, practices, and policies with application in complex situations.
Team Work:
Ability to lead collaborative teams for larger projects across functional areas, impacting management and operations of multiple organizational areas.
Customer Service:
Provide high‑level service and staff training; resolve service issues promptly and respectfully.
Employees in this job must be competent to provide patient care to the following age groups: Young adult (16–30), Middle Age (30–60), Elderly (60+).
Physical Nature of the Job: Medium work; may need to exert up to 50 lbs occasionally, or up to 20 lbs frequently; stands/moves throughout shift.
