Case Manager - Rapid Re‑Housing
Join Community Missions, Inc. as a Rapid Re‑Housing Case Manager and help individuals and families transition from homelessness into safe, permanent housing. You’ll provide move‑in support, manage housing assistance funds, and deliver stabilization services that give clients the tools to remain housed long‑term.
Key Responsibilities
Conduct housing assessments and create individualized re‑housing and stabilization plans.
Assist clients with securing housing, negotiating with landlords, and coordinating move‑in activities.
Provide case management focused on budgeting, employment, education, and family support.
Advocate with landlords/utility companies to reduce barriers.
Process financial assistance (security deposits, rent, utilities) and maintain accurate HMIS/client files.
Collaborate with the Rapid Re‑Housing Team and community partners to ensure client success.
Qualifications
Bachelor’s degree in a health service related field plus 1 year professional experience in the human services field preferred.
Valid New York State Driver’s License with a clean driving record.
First Aid/CPR trained (agency will provide).
Proficiency with MS Office; experience with case management software a plus.
Strong communication, organizational, and advocacy skills.
