Overview Senior Residence Counselor responsible for implementing trauma‑informed, person‑centered care plans and ensuring a safe, supportive environment for adult residents at Community Missions of Niagara Frontier.

Key Responsibilities

Conduct assessments of residents’ strengths and needs.

Develop and implement individualized care plans that address resident needs and goals.

Monitor resident progress through regular follow‑ups and adjust care plans as needed.

Advocate for residents to ensure they receive appropriate services and support.

Maintain accurate and up‑to‑date case files and documentation in compliance with regulatory requirements and policies.

Assist residents with daily living activities, such as meal preparation, medication management, and cleaning.

Provide crisis intervention and support to residents in emergency situations.

Ensure compliance with all relevant policies, procedures, and regulations.

Participate in on‑call rotation with a team.

Work evenings, weekends, and holidays.

Qualifications

Bachelor’s degree in psychology, social work, counseling, or a related field plus 1 year of professional experience in human services.

Valid New York State Driver’s License with a clean driving record.
