10/29/2025 1800 DaVita Integrated Kidney Care (IKC) is looking for a Full-Time Nurse Practitioner (NP) to join us in XXXX. As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You’ll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients. The NP provides comprehensive healthcare, both in-person and via telehealth, across the many co-morbid chronic conditions common to CKD/ESKD patients within a designated market or geographical region. The NP functions autonomously, yet in collaboration with all members of the healthcare team to provide, coordinate, and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for their patients. The NP assumes a clinical leadership role and coordinates activities with the healthcare team and healthcare systems to meet the health needs of their patients. Location and Schedule Location: Hybrid-Remote. Work from home (telehealth) and travel within an assigned geography including Dialysis Clinics and Nephrology Practices. Monday–Friday schedule with the ability to accommodate patient and nephrology partner’s availability. Must be flexible. Full-Time self-managed schedule No nights, weekends and on-call Position Details: Travel within an assigned geography including Dialysis Clinics and Nephrology Practices. Focus on objective outcomes such as delayed disease progression, reduced hospitalizations, and optimized health care utilization.
Responsibilities
Performs enhanced annual wellness visits or Comprehensive Health Evaluations (CHEs) to identify and evaluate patients’ co-morbid conditions and assist with patients’ needs, including medical, social, emotional, and financial concerns through medical record review, consultation with patient, family, and healthcare team members.
Manage complex CKD/ESKD patients with the aim of delaying disease progression, reducing hospitalizations, and improving health outcomes; leverage prescriptive authority, medication management, ordering/interpretation of labs/diagnostics, and collaboration with other providers.
Provide care coordination and post-discharge planning and follow-up with high-risk patients to prevent readmissions.
Collaborate autonomously within established protocols and evidence-based guidelines while practicing with clinical leadership.
Qualifications
Passion for delivering care to patients with complex, chronic illnesses
Minimum of two (2) years' experience as a Nurse Practitioner (NP)
Current unrestricted NP license to practice in the state of practice (NJ required; NY preferred but able to obtain upon hire)
Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
Current CPR or BLS certification
Current valid driver’s license and insurance in the state of residence
Intermediate computer skills with proficiency in MS Word, Excel & Outlook
Demonstrated proficiency in clinical assessment and management of complex comorbidities
Strong clinical judgment and autonomous decision-making within established protocols
Home Office, high speed internet, and reliable transportation required
Preferred Qualifications
Experience in Value Based Care or Population Health including documenting and coding from a Medicare Advantage risk perspective and Health Risk Assessments on high-risk/chronically ill patients
5+ years’ experience as a Family NP, Primary Care NP, Internal Medicine, Cardiology, Nephrology or Case Management
Experience with population health management and Cerner or similar EHR systems
