The clinical pharmacist provides comprehensive medication management services that integrate clinical services, order management, patient/staff education, and research. Clinical pharmacists are responsible for improving the patient’s experience, safety, and achieving positive clinical and financial outcomes. This position reports to an immediate supervisor/manager.

Major Responsibilities

Provides medication management services in a collaborative manner that includes reviewing patient medication history, educating patients on drug therapy, establishing desired outcomes, developing therapeutic plans, monitoring patient outcomes, and documenting medication‑related issues in the electronic health record.

Maintains and updates patient profiles with demographics, diagnosis, allergies, and medications.

Monitors drug therapy to evaluate appropriateness of use, dose, regimen, therapeutic duplication, and drug interaction.

Ensures timely and accurate dispensing of medications and proper preparation of pharmaceutical products, including compounding, chemotherapeutic, hazardous, and investigational agents.

Promotes use of formulary drugs, coordinates procurement, and conducts drug usage evaluations to identify cost avoidance and reduce waste.

Detects, monitors, documents, and reports adverse drug reactions and medication errors.

Participates in Joint Commission and other accreditation activities; contributes to quality improvement plans.

Precepts IPPE/APPE rotation students and pharmacy residents; prepares educational materials for the healthcare community.

Position Qualifications

Doctor of Pharmacy (Pharm.D) degree or Bachelor of Science in Pharmacy (BSPharm) degree with at least 3 years of hospital experience.

Licensure with the Massachusetts Board of Registration in Pharmacy.

Preferred: ASHP Accredited Residency in Pharmacy Practice (PGY1); D. degree with minimum 1 year of hospital experience.
