Position Overview The Pediatric pharmacist provides medication management, education, and quality improvement in the pediatric ICU and pediatric general medicine units. The role also supports operations, financial stewardship, safety, regulatory compliance, research, and education activities.

Major Responsibilities

Provide patient‑centered medication management services, including reviewing histories, educating patients, establishing outcomes, developing therapeutic plans, monitoring laboratory data, and documenting interventions.

Maintain accurate patient profiles with demographics, diagnoses, allergies, and current medications.

Monitor drug therapy for appropriateness of dose, route, regimen, duplication, and interactions; provide drug information to providers and patients.

Perform clinical review and verification of medication orders; enter orders when required; discuss clarifications with prescribers.

Facilitate medication discharge planning and transition to alternate sites of care.

Ensure timely, accurate dispensing of medications; prepare oral, non‑sterile, sterile compounding, chemotherapeutic, hazardous, and investigational agents.

Maintain regulatory compliance for controlled substances and supervise pharmacy technician work.

Promote formulary use, manage medication inventory, and identify cost avoidance opportunities.

Detect, document, and report adverse drug reactions and medication errors; participate in accreditation and quality improvement activities.

Precept pharmacy students and residents; develop educational materials; participate in continuing education and outreach activities.

License/Certification/Education Required

Doctor of Pharmacy (Pharm.D.) degree; or Bachelor of Science in Pharmacy (BSPharm) with 3 years of hospital experience.

Licensure with the Massachusetts Board of Registration in Pharmacy.

Preferred

ASHP Accredited Residency in Pharmacy Practice (PGY1) is recommended.

Pharm.D. degree with a minimum of 1 year of hospital experience.

Equivalent education and experience may be considered for those who meet proficiency requirements listed above.
