Enhanced Support Practitioner The closing date is 16 November 2025

The post holder will work in close collaboration with other professionals, including the wider Multi‑Disciplinary Team (nursing, psychiatry, psychology and speech and language therapy) in assessing service user needs, devising care plans and evaluating outcomes that promote service user empowerment through the process.

The post holder, with the wider Multi‑Disciplinary Team will provide high quality, individualised care, which addresses the needs of the service users and meets individual health needs.

The Intensive Support Service will provide intensive support to adults with learning disabilities who are presenting challenging behaviours and/or mental health problems in the community to prevent, where possible, admission to hospital. The team will act as a gateway to the inpatient assessment and treatment service and work in close liaison with inpatient services to facilitate appropriate admission and timely discharge from services. The team will provide an 8am‑8pm service Monday to Friday, 9am‑5pm Weekends and Bank Holidays and will work with the service user in a variety of health and social care settings.

Main duties of the job Maintain responsibility for a caseload of people with learning disabilities and health needs, providing a holistic approach to service user assessment, treatment, review and management of packages of care.

Ensure that the statutory requirements of the Mental Capacity Act are observed and applied.

Have the clinical skills relative to the delivery of care in the service area.

Have effective working relationships with other professionals within the service area, across the Strategic Business Unit, Trust and other NHS providers and key stakeholders.

Contribute towards effective management and utilisation of resources within the service area.

Provide clinical advice for service users who present with challenging and mental health needs that promote and facilitate the positive health and wellbeing of people with learning disabilities, including direct work with service users, carers, colleagues and other service providers.

Job responsibilities In conjunction with carers and other MDT members, the post holder will lead in the assessment, planning, implementation and evaluation of care for an identified group of service users.

To recognise and promote all aspects of equality and diversity whilst assessing, planning, delivering and evaluating care.

To carry out clinical procedures/techniques whenever required in accordance with the policies and procedures of the Trust.

To implement and promote evidence based clinical processes and standards of practice across the service area.

To undertake a range of comprehensive and evidence based clinical risk assessments and associated care plans that are relevant to the service users care needs.

To review and communicate service users risk assessments and management care plans in accordance with Trust policy, ensuring meaningful service user, carers and MDT involvement throughout the process.

To provide written reports as necessary for service user reviews, in accordance with agreed record keeping standards.

To ensure that the administration and safe custody of drugs and medications are in accordance with Trust policy and procedure, and the NMC guidelines where relevant.

To regularly assess and report on the side effects experienced by service users.

To lead specific groups / sessions with clear objectives where identified.

Person Specification KNOWLEDGE, TRAINING AND EXPERIENCE

Relevant Professional Registration with Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC)

Teaching/training experience

Additional areas of training through short courses o ENB 998/Mentorship (Mentoring & Preceptorship) or equivalent o Post graduate qualification

Understanding of the health and well‑being needs of people with a learning disability over a broad range of health issues

AREAS OF EXPERIENCE AND KNOWLEDGE

Organisational skills and the ability to work effectively within a management structure

Communication skills — written, verbal and non‑verbal

Experience of assessing and managing clinical risk

Experience in care planning

Leadership and motivational skills

Provision of effective clinical supervision

COMMUNICATION SKILLS

Recognises and reflects on barriers to effective communication, modifying communication in response

Keeps accurate and complete records and communication consistent with legislation, policies and procedures
