Description
Educates patients on cancer risk and screening community residents. Guides clinic patients in obtaining cancer screening tests and assist with barriers to care. Has direct interaction with professional staff and community-based organizational members.
ESSENTIAL FUNCTIONS
(include, but not limited to the following)
Maintains detailed records to measure effectiveness of program and suggest ways to improve throughout the process. Schedules follow-up appointments and provide information and reminders to patients & physician offices. Tracks outcomes of referrals and treatments Coordinates information among referring, primary care and consulting physicians and ancillary departments. Participates in mandatory in-services related to moral, ethical and legal issues. Performs patient and family education based on established standards as required by age of patient, and documents education in Electronic Health Records. Recognizes the dignity or worth of individuals, respects cultural, spiritual and psychosocial differences and apply philosophical and ethical concepts that promotes equality and continuity of care. Identifies community sites presentations. Conducts educational sessions regarding colorectal cancer prevention (lifestyle) and screening and conducts follow-up Reviews and evaluates teaching methods and work activities. Prepares assignments, curriculum, classroom activities, or other materials. Assists patients in clinic to obtain colorectal cancer screening Keeps logs of time spent to recruit sites, run the group classes and implement navigation for cost effectiveness. Attends monthly team meetings. Integrates culturally sensitive communication in all interactions with patients and community. Performs other duties as assigned. Requirements
Two years of instructional or subject work experience with patients; and/or two years of progressively responsible instructional or work experience with patients in a Native American healthcare setting One year clinical experience, preferred
