General Description

The core responsibility of the Service Coordinator is to assist and synchronize supports and services with the people we serve in a dynamic and regulated environment. Service Coordinators build meaningful relationships with people, families, and community providers while completing essential duties around service and care planning and monitoring. Service Coordinators remain agile, aligned with TRE’s Mission, Vision, and Values, and steward TRE’s financial resources.

Essential Duties / Responsibilities

Promotes excellent customer service and TRE values through intentional communication strategies. Develops understanding of Long Term Care (LTC) rules and regulations to maintain compliance and support the people served. Organizes and implements service plan development tasks in person, by phone, or through technology to meet needs while staying compliant with policies and procedures. Monitors authorized services and utilization to promote person-centered practices, ensuring health, safety, and autonomy. Cultivates understanding of local and extended community resources to support the people we serve. Attends departmental and agency meetings and trainings to maintain and improve performance, knowledge, skills, and abilities. Conducts service plan development tasks in community settings with transportation provided by you, including private homes, group homes, facilities, and community locations; daily/weekly. Perform driving responsibilities as required and maintain driving eligibility with valid CO driver’s license and appropriate insurance. Other Duties / Responsibilities

Support team members with coverage of service plan development tasks to maintain quality and continuity of services. Participate in professional goal creation and career development plans, reviewed at least annually. Adapt to changes in state and federal rules and regulations to support TRE and the people served, as needed. Assume other duties to promote the health and wellbeing of people served and TRE, as needed.

Job Qualifications

Knowledge, Skills and Abilities:

TRE advocates for person-centered case management. Service Coordinators must learn and adhere to best practices in person-centered thinking. Proficient with computer-based technology: Apple devices, Microsoft 365 (TEAMS, Word, Excel, PowerPoint, SharePoint, OneDrive), Outlook, Skype for Business, Adobe Acrobat, and related tools; familiarity with state/proprietary databases for accurate records. Solid understanding of case management best practices. Ability to type accurately and manage multiple tasks with attention to detail. Friendly disposition, positive attitude, and professional interactions. Empathy for people with long-term care needs. Strong problem solving, critical thinking, and organizational skills. Ability to work independently and meet deadlines while prioritizing emergencies. Cultural awareness and understanding of biases in the workplace. Education and Experience Requirements

Minimum:

Bachelor’s degree in a human behavioral science or related field. If the educational requirement is not met, relevant LTSS experience may be considered in lieu of education. Preferred:

Knowledge of Medicaid Waiver programs and experience with persons with disabilities; familiarity with community resources and resource development. Working Environment and Physical Activities

Office, community, private residences, commercial properties, and virtual environments. Moderate physical activity; ability to walk, sit, stand, and lift up to 20 pounds.
