Position Title Enrollment Coordinator

Department CMA – Operations and Communication

Reports To Director of CMA Operations and Communication

FLSA Status Non-Exempt

Starting Range $22.32 – $23.18 hourly (Rates reflect FY24-25 and are subject to change; contact hr@tre.org to confirm current rates.)

General Description The CMA Enrollment Coordinator plays a vital role in facilitating access to long-term services and supports for individuals of all ages with disabilities or complex care needs. Working within a regulatory framework shaped by the Colorado Department of Health Care Policy and Financing (HCPF), this role requires the ability to adapt to evolving systems, policies, and service delivery models. Coordinators engage directly with individuals, families, and provider networks to guide them through program eligibility and enrollment.

This position calls for a proactive, detail-oriented, and empathetic professional who can successfully navigate changing technologies and workflows, deliver quality service, and maintain compliance with federal and state regulations.

Essential Duties and Responsibilities

Provide exceptional customer service by responding to all inquiries (phone, email, in-person) within 48 hours, maintaining a professional and supportive presence.

Complete enrollment processes for a wide variety of Home and Community-Based Services (HCBS) waivers and state-funded programs, including CES, SLS, DD, CHCBS, CHRP, EBD, CLLI, CMHS, and BI.

Track, update, and document all activities and status changes using state and agency data systems, including the BUS, Bridge, CCMS, Trails, and other evolving HCPF platforms.

Ensure enrollment is completed within 90 days; if not, document the cause and inform a supervisor. When applicable, assume responsibility for completing additional requirements such as 6‑month reviews, Continued Stay Reviews, and monitoring.

Submit timely and accurate referrals to provider agencies, nursing facilities, and community-based organizations based on individual needs and availability.

Coordinate with County Departments of Human Services (El Paso, Teller, Park, Pueblo), families, and Medicaid to establish and maintain financial eligibility.

Conduct warm hand‑offs to Service Coordinators for long‑term care, ensuring all enrollment documentation and client needs are properly transitioned.

Perform monitoring tasks and ULTC 100.2 functional assessments when needed to determine ongoing eligibility.

Stay informed of ongoing HCPF system changes and policy updates through required trainings and internal briefings.

Collaborate with internal departments—such as Quality Enhancement, Navigation, Enrollment Services, Child & Family Services, and Administration—to support the client journey.

Complete and maintain all required documentation and case notes according to CMS, HCPF, and TRE standards.

Be available during regular business hours and adjust schedule as needed to meet client and team needs.

Maintain all driver and vehicle eligibility standards, including a valid license, insurance coverage, and reliable personal transportation.

Participate in regular supervision meetings, complete required reports and updates, and contribute to team meetings and goals.

Other Duties and Responsibilities

Utilize mobile technology tools, including cell phones and laptops (Surface Pro), with proficiency in Windows 10, Microsoft Office, Skype, and other cloud‑based systems.

Participate in training, committees, conferences, or other professional development activities to stay informed and expand expertise.

Maintain consumer records and documentation in accordance with internal and external standards.

Support team coverage as needed across service areas.

Perform other related duties as assigned, especially in response to evolving HCPF processes and agency needs.

Qualifications Minimum Requirements

Bachelor’s degree in a human services‑related field, or a combination of education and 5 years of documented, verifiable human services experience.

Strong communication, organization, and advocacy skills.

Ability to adapt to system‑wide changes and work effectively across diverse environments.

Preferred Qualifications

Bachelor’s degree in Sociology, Psychology, Education, Human Services, or closely related field.

Experience in a case management or Medicaid waiver program setting.

Knowledge of family systems, disability services, and community resource navigation.

Familiarity with HCPF systems (BUS, Trails, Bridge, CCMS) and regulatory requirements.

Tools and Equipment Used Computers/tablets, cloud-based systems, printers, telephones, personal vehicles, etc.

Working Environment / Physical Activities Work is performed in a variety of settings including office, home, virtual, and community locations. Moderate physical activity such as walking, sitting, standing, and occasional lifting (up to 20 lbs) is required. A valid Colorado driver’s license (unless exempt due to military status), reliable personal transportation, and minimum insurance coverage of $100,000/$300,000/$100,000 are required.
