This full-time, onsite role is embedded in the Springfield District Courthouse and focuses on serving adults at the initial court hearing/arraignment stage to prioritize and expedite treatment and recovery services. The Court Social Worker works closely with BHN's Project HOPE Outreach Team, Co-Response Team, BHN Mobile Crisis, and the Springfield Police Department to ensure warm hand-offs and support continuity of care. What You'll Do Identify adults likely eligible for access-to-care services by coordinating with probation officers and attorneys before arraignment sessions Meet with eligible participants pre- or post-arraignment to introduce services and offer enrollment Conduct psychosocial assessments and triage immediate treatment/resource needs (e.g., overdose, harm to self/others, community safety) Develop initial service plans in collaboration with participants and facilitate prompt access to care Obtain releases and communicate treatment recommendations to courts and external stakeholders, as appropriate Provide referrals, care coordination, and transportation assistance to connect participants with services Build partnerships with community-based service providers to enhance engagement and long-term success Participate in multidisciplinary meetings, including BHN team meetings, HCAT, and Springfield Police Department coordination calls Maintain HIPAA-compliant documentation, including assessments, treatment plans, safety plans, and progress notes Collect and enter data for program evaluation and support reporting for COSSUP grant deliverables What We Offer Mission-Driven Impact

- Work closely with law enforcement, court systems, and healthcare providers to drive real outcomes Who You Are Bachelor's Degree in Social Work, Counseling Psychology, or related field (required) LADC II or CADAC certification (preferred) 3-5 years' experience working with adults with co-occurring substance use and mental health disorders (preferred) Minimum 1 year of experience working with adults experiencing chronic homelessness (required) Strong knowledge of Hampden County resources and recovery supports (required) Familiarity with evidence-based, trauma-informed approaches to substance use and mental health treatment Experience working in court systems (preferred); bilingual Spanish a plus Highly organized with the ability to work both independently and within a multidisciplinary team Valid driver's license, reliable vehicle, and willingness to travel (required) How to Apply
