Recovery Counselor - Recovery Services (South State) Schedule: Monday - Friday; 8:00 am - 4:30 pm

Location: SOUTH STATE

The Recovery Counselor will assess the client’s personal, medical, emotional, social, and environmental situation to plan for treatment through home, community, and office visits. The Counselor will provide individually‑based motivational treatment and support to help clients recover from mental illness, act as an advocate, link them to community services, and deliver client‑centered, strengths‑based, trauma‑informed, harm‑reduction services for clients with severe mental illnesses and co‑occurring substance abuse/addiction disorders.

$22.89 – $26.32 /hr $1,500 Sign‑On Bonus!

Responsibilities

Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.

Coordinate the establishment of an individual recovery plan with the client, client’s support system, and other care providers.

Complete daily progress notes in a timely manner according to Trilogy’s documentation policy and agency standards.

Document client care in all required systems (IMCANS, LOCUS, EMR tracking forms).

Recognize opportunities to move clients to appropriate levels of care and provide referrals as needed.

Assist clients in identifying signs and symptoms of decompensation, assess crisis situations, and facilitate stabilization or hospitalization as appropriate.

Provide psychoeducation, medication training, and monitoring per Trilogy policy.

Coordinate with nurse/pharmacy to ensure accurate medication administration.

Document medication administration in real time on the MAR.

Help clients develop independent living skills (hygiene, housekeeping, nutrition, shopping).

Assist clients in applying for entitlements (SSI, SSDI, Medicare, Medicaid, LINK).

Manage client money and support representative payeeship processes per policy.

Accompany and transport clients to appointments and support housing, substance use, budgeting, social support, and medication needs.

Advocate on behalf of clients and empower them to self‑advocate.

Communicate effectively within a multidisciplinary team and participate in electronic team communication.

Engage in daily team meetings for shared caseload collaboration.

Provide on‑call and crisis coverage as assigned.

Perform related duties and projects as assigned.

Qualifications

One of the following:

Bachelor’s degree in counseling, guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or a related human service field.

Bachelor’s degree in any other field with two years of supervised clinical experience in a mental health setting.

Licensed Practical Nurse under the Nurse Practice Act (IL 225 ILCS 65).

Certificate of Psychiatric Rehabilitation from a DHS‑approved program, high school diploma/GED, and two years’ experience in a mental health setting.

CRSS certification through IAODAPCA.

Family Partnership Professional certificate in good standing with IAODAPCA.

Occupational Therapy Assistant licensed under the Illinois OT Practice Act (IL 225 ILCS 75) with one year of mental health experience.

High school diploma or GED with five years of supervised clinical experience in a mental health setting.

Valid Illinois Driver’s License with daily vehicle access; must hold $100,000/$300,000 auto liability insurance.

Experience working with individuals with mental health and/or substance abuse diagnoses preferred.

Experience in community‑based services preferred.
