Shift: Monday - Friday | 7:00 AM - 3:30 PM (On-call/Weekend Availability)
Setting: Inpatient Skilled Nursing Facility (SNF)
Job Summary & Responsibilities: Conduct evaluations and diagnose speech, language, cognitive, and swallowing disorders. Develop and implement evidence-based treatment plans tailored to patient needs. Provide direct therapy services to improve communication and functional independence. Collaborate with physical therapists, occupational therapists, nursing staff, and physicians for holistic patient care. Educate patients, families, and caregivers on therapy goals and home care strategies. Maintain accurate and compliant clinical documentation. Participate in discharge planning, care conferences, and quality improvement efforts. Education, Licensure, and Experience:
Master's degree in Speech-Language Pathology from an accredited program. Current state California SLP license (or eligibility for licensure). Certificate of Clinical Competence (CCC-SLP) from ASHA preferred; CFs will be considered. Prior experience in a skilled nursing or inpatient rehabilitation setting preferred, but not required.
