Occupational Therapist (New Graduates Accepted) – Walnut Creek, CA

Full time & Part Time
Schedule:
Monday-Friday (7:00 am - 3:30 pm) with on-call/weekend requirement.
Setting:
Inpatient Skilled Nursing/Post‑Acute Rehabilitation.
Patient Ratio:
10‑14 Patients
Key Responsibilities
Evaluate residents' physical, cognitive, and functional abilities to determine treatment needs.
Develop and implement individualized care plans focusing on activities of daily living (ADLs), adaptive techniques, and safety awareness.
Provide direct therapy interventions to enhance mobility, strength, coordination, and independence.
Recommend and train residents in the use of adaptive equipment and assistive devices.
Collaborate with interdisciplinary teams including physical therapists, speech therapists, nursing staff, and physicians to ensure coordinated care.
Qualifications / Experience
Master's or Doctorate in Occupational Therapy from an accredited program.
Current and valid California Occupational Therapist (OT) License in state CA.
Prior experience in a Skilled Nursing Facility (SNF) or post-acute care setting.
Familiarity with rehabilitation documentation systems (e.g., NetHealth, Casamba, Rehab Optima).
New Graduates are also accepted.
