Job Description
We are seeking a skilled Registered Nurse with a focus on Case Management. This position requires a collaborative approach to patient care, ensuring effective resource utilization and care coordination in an acute care setting.
Responsibilities
Manage and coordinate patient care within an acute care environment. Work collaboratively with interdisciplinary teams to optimize patient outcomes. Facilitate discharge planning and ensure continuity of care. Educate patients and their families regarding care plans and resources available. Participate in quality improvement initiatives and contribute to policy development. Required Experience / Certifications / Licensure
Minimum of 2 years of Case Management experience in an acute care setting. Current BLS certification required. Florida or Compact nursing license required, with a recent Nursys record on profile.
