Job Description

The Integrated Case Manager for Population Health is an integral member of the patient-centered care team, responsible for the assessment, planning, facilitation, care coordination, evaluation, and advocacy for patients' and families' comprehensive healthcare needs. This role focuses on enhancing patient safety, quality of care, and cost-effective outcomes, particularly for patients who have experienced complex health challenges requiring extensive resource management.

Responsibilities

Conduct comprehensive assessments of patient and family needs. Develop personalized, patient-centered care plans. Engage with patients and families, promoting advocacy and ongoing communication. Utilize critical thinking and motivational interviewing techniques to assist patients in overcoming barriers. Provide counseling related to treatment decisions and end-of-life issues, including Advanced Care Planning. Ensure seamless transitions between care settings. Collaborate with the treatment team to manage complex medical and social needs. Evaluate patient responses and modify care plans as necessary. Participate in process improvement activities. Facilitate interventions for cases involving abuse and neglect. Required Experience / Certifications / Licensure

Minimum 5 years of Acute Inpatient Case Management experience. Strong computer skills, including Epic EMR experience. Nursing degree or MSW with current valid Michigan licensure. BLS certification through AHA. Recent acute inpatient case management experience within the last two years. Why ARMStaffing?
