Full-Time MDS Coordinator RN/LPN .

Job Responsibilities:
Complete SNF MDS assessments and transmit accordingly.
Oversee assessment processes, set assessment schedules, and ensure assessments are done accurately and timely.
Participate in discharge planning.
Observe and report responses to interventions.
Communicate with patients, families, caregivers, and healthcare team to promote optimal care.
Complete documentation supporting services provided, meeting organizational, clinical, and reimbursement requirements.
Follow infection control guidelines.
Contribute to quality improvement initiatives.
Qualifications:
Current RN or LPN license in Louisiana.
At least 2 years of MDS experience in long-term care.
Current CPR certification.
