Responsibilities The MDS Coordinator LPN supervises and leads Unit Secretaries, Certified Nurse Assistants & Licensed Practical Nurses in providing quality of care for patients of all age groups by performing basic nursing tasks. Facilitates workflow practices by providing support services as required by the nursing staff.
Completes SNF MDS assessments and transmits accordingly.
Oversees assessment processes, sets assessment schedules, and assures assessments are done in an accurate and timely manner
Observes and reports responses to interventions as assigned
Communicates with patient, family, caregivers, significant other, and members of the healthcare team to promote maximum benefit of care.
Completes all documentation required to support the services provided and meet organizational, clinical and reimbursement requirements.
Follows infection control guidelines as per nursing policy and procedure
Responds to department needs. Participates in and contributes to the quality improvement process for the department and the institution.
Qualifications
Graduate of an accredited licensed practical nursing program.
Current Practical Nursing License (LPN) in the State of LA.
MDS experience in long term care required
Current CPR certification
