Job Description
Primary Duties and Responsibilities Functional Medicine Health Coach/Nutritionist/Dietician partners directly with an Institute of Functional Medicine (IFM) certified Hoag Physician and Exercise Physiologist to form a novel care team specializing in preventive treatment, integrative care and nutrition counseling. The physician and care team are on the forefront of "root-cause" treatment and care while simultaneously providing best-in-class client service. Utilize monitoring and evaluation techniques that consider the client's specific nutrition needs. Read and interpret complex labs to provide custom recommendations and nutrition/diet planning to improve overall health and wellness. Familiarity with the following labs: NutrEval FMV, GI Effects or GI-MAP, DUTCH Plus, IgG Food Sensitivity Panel, etc. Utilizes medical history, laboratory data, drug profile, anthropometric measurements and diet history to evaluate for nutrition-related consequences. Consider environmental factors and past medical history to evaluate the whole person. Recommend food choices to heal underlying ailments, i.e., gut issues, etc. Coordinate with physician and staff to develop meal plans, while considering cost and client preferences. Complete necessary documentation and assessments of clients. Review dietary intake for factors that affect health conditions and nutritional risk. Evaluates psychosocial, functional and behavioral factors related to food access, selection, preparation, physical activity and understanding of health condition. Provides appropriate follow-up in accordance with the client's treatment plan goals. Ability to be caring and empathetic when helping clients address dietary issues and related emotions. Provide dietary services in accordance with company guidelines, state and federal laws and regulations. Ensures that care delivered to each patient is in accordance with the state of California dietetics regulations, licensing, certification or registration. Participates in treatment team meetings to contribute interventions and assessment results to the multidisciplinary team. Identifies and reports opportunities for quality improvement specific to dietary/nutritional services to leadership. Provides educational group sessions to patients when scheduled on dietary issues and healthy nutrition. Qualifications
Education and Experience
Bachelor's degree in Nutrition, Dietetics or related field required 2+ years in applied functional medicine; hormone balancing and stem cell applications. Certifications Required
Registered Dietitian Nutritionist (RDN) Certifications Preferred
Certified, or dedicated to pursuing certification with A4M/MMI and/or Institute of Functional Medicine
