Associate Director, Nursing Care Management

Community of Hope is seeking an Associate Director, Nursing Care Management to make a lasting impact in the lives of patients who need it most. As the leader of COHs Nursing Care Management Program, youll ensure those with chronic conditions or special health needs receive compassionate, coordinated care. Youll inspire your team, strengthen quality outcomes, and help reduce unnecessary hospital visits through proactive, patient-centered support. This position is located at our Conway Health and Resource Center in SW DC. Salary: $110,000 $115,000. What Youll Do Lead the nurse care management team in managing the effectiveness of the care management service delivery model, including evaluation of program outcomes and individual staff productivity and performance. Ensure that patients with chronic diseases or unnecessary utilization are adequately monitored and supported using population health management tools. Identify gaps in services and assist in developing plans and partnerships to meet patient needs. Oversee coordination of transitions between healthcare providers and settings to reduce emergency department and inpatient admissions, readmissions, and length of stay. Oversee regular communication with panel of care?managed patients via face?to?face or telephone encounters based on level of acuity and program requirements, as well as via the patient portal. Meet patients where necessary to accomplish this goal. Facilitate ongoing communication among practice and care team by ensuring participation in huddles, hosting regular conference calls, in?person meetings, or coordinating regular email updates to ensure alignment of activities and exchange of information across the care teams. Coordinate with Nurse Director of Clinical Services, Medical Directors, and departmental leads as needed, including management of clinically related patient complaints, unusual incident reports, HIPAA and OSHA incidents.

Requirements Must?Haves Bachelor of Science degree in nursing required. Current, unencumbered DC Registered Nurse license with current BLS certification required. Knowledge of primary care and health maintenance required. One year management experience required. Ability to work with computers and electronic health records required. Strong verbal and written communication skills required. Strong organizational skills with an ability to multitask required. Demonstrated cultural competence in communicating with low?income populations required. Demonstrated ability to function effectively in a team required. Ability to travel between COH sites or relocate to a different COH site on a full or part?time basis required. Ability to work Saturdays and evenings required.

Nice?to?Haves Three years of care management experience preferred. Experience with educating patients with chronic health conditions preferred. Bilingual in Spanish, Amharic, or French preferred. Familiarity with eClinicalWorks. Nonprofit or mission?driven healthcare experience.
