Position Summary Clinical Pharmacist, Ambulatory Care (East Beltline & Browning Claytor Offices). Responsible for providing clinical pharmacy services in an outpatient and inpatient practice area, including drug therapy management, formulary support, pharmacy education, and quality improvement and research related to drug therapy.

Qualifications

Doctor of Pharmacy (PharmD) from an ACPE-accredited pharmacy school (required).

PGY‑1 Residency (required) and PGY‑2 Ambulatory Care Residency strongly preferred or equivalent practice experience in ambulatory care with direct pharmacotherapy management responsibilities.

BPS Specialty Certification in Pharmacotherapy or Ambulatory Care preferred; certification within one year of hire acceptable.

Licensed pharmacist and controlled substance pharmacist in the State of Michigan, or eligibility to be licensed.

What you will do

Full‑time (80 hours per pay period), salaried Monday‑through‑Friday schedule as a Clinical Pharmacist, Ambulatory Care within the Trinity Health Medical Group primary care PCMH offices in Grand Rapids, Michigan.

Implement disease management under institution‑approved collaborative practice agreements (e.g., diabetes, hypertension, hyperlipidemia, asthma, COPD, anticoagulation).

Conduct referral‑based Comprehensive Medication Reviews and provide recommendations to physicians.

Provide daily ad‑hoc informal consultations to improve pharmacotherapy effectiveness, safety, and cost.

Participate in team‑based care within a multidisciplinary model.

Support Anticoagulation Management Service on an as‑needed basis.

Collaborate with the infectious disease team on antimicrobial stewardship.

Perform pharmacy unit inspections.

Engage in training and precepting of Trinity Health Grand Rapids PGY‑1 & PGY‑2 Ambulatory Care Pharmacy residents and PharmD students from Ferris State University, and participate in medical group and/or health system committees.

Manage drug therapy, identify and resolve drug‑related problems, assist physicians with dosing regimens, and communicate with clinicians to develop policies, protocols, and guidelines.

Support the Pharmacy & Therapeutics Committee, including formulary drug reviews and medication usage evaluations.

Participate in performance improvement initiatives and research, and serve on relevant committees as a leader or member.

Provide education on pharmaceutical care and drug therapy to patients, staff, physicians, nurses, and other clinicians.

Perform other related duties as assigned (e.g., quality assurance reports, regulatory inspection preparation).
