Clinical Pharmacist, Ambulatory Care (East Beltline & Browning Claytor Offices)
Employment Type Part time
Shift Monday thru Friday, full‑time (80 hours per pay‑period)
Job Description Responsible for providing clinical pharmacy services in an outpatient and/or inpatient practice area, managing drug therapy and assessments for patients within the practice area. Supports established programs including drug formulary, pharmacy education, quality improvement, and research related to drug therapy.
Responsibilities include disease management within institution‑approved collaborative practice agreements (diabetes, hypertension, hyperlipidemia, asthma, COPD, anticoagulation, others), referral‑based Comprehensive Medication Review with recommendations to physicians, Transition of Care visits, daily ad hoc informal consultations to improve pharmacotherapy effectiveness, safety, and cost, and participating in team‑based care within a multidisciplinary model. Additional activities include time in our Anticoagulation Management Service, antimicrobial stewardship partnership with our ID team, pharmacy unit inspections, and training of Trinity Health Grand Rapids PGY‑1 & PGY‑2 Ambulatory Care Pharmacy residents and PharmD students from the College of Pharmacy at Ferris State University. Participation in medical group and/or health system committees is also expected.
What you will need
Doctor of Pharmacy degree (PharmD) from an ACPE‑accredited school/college of Pharmacy (required).
PGY‑1 Residency (required) and PGY‑2 Ambulatory Care Residency (strongly preferred) or commensurate practice experience in ambulatory care with direct pharmacotherapy management responsibilities.
BPS Specialty Certification (Pharmacotherapy or Ambulatory Care) preferred; eligible with certification within 1 year of hire.
Licensed pharmacist and controlled substance pharmacist in the State of Michigan, or eligible.
What you will do
Provides drug therapy management and assessment.
Identifies and resolves potential and actual drug‑related problems.
Assists physicians in designing dosing regimens based on patient‑specific dosing parameters.
Communicates and collaborates with physicians and other clinicians to develop drug therapy policies, protocols, and guidelines.
Provides support to the Pharmacy & Therapeutics Committee, including formulary drug reviews and medication usage evaluations.
Participates in performance improvement initiatives and research.
Participates in departmental, multidisciplinary, hospital and health‑system committees as leader, member, or consultant.
Participates in the training, education, precepting, and evaluation of pharmacy students and/or residents.
Provides education and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians, nurses, students, and other clinicians.
Performs other related duties as assigned, such as quality assurance reports and preparation for regulatory inspections.
