Lead Housing Case Manager at Mental Health Systems

Base Pay Range $31.00/hr - $32.00/hr

Job Summary Work with community organizations, government agencies and property owners to secure living arrangements, negotiate rents, deposits, and move‑in terms on behalf of clients. Manage lease agreements for short‑term, transitional, and permanent housing.

Responsibilities

Conduct frequent housing inspections, follow reporting requirements, and work with clients to remediate issues.

Provide case consultation, crisis intervention, discharge placement, and targeted case management services to assist clients in meeting basic needs and housing needs.

Complete accurate and timely documentation as contractually required and meet minimum requirements for direct billable client services.

Locate housing opportunities to meet short‑term, transitional, and permanent housing needs.

Coordinate, oversee, and educate clients on lease agreements for client housing.

Prepare subsidy applications, maintain client housing files, and ensure rent calculations are accurate.

Assist in monthly tenant rent collection, transport clients to housing locations and help them secure required documents for housing placement.

Conduct housing screenings and assessments utilizing the coordinated entry system required.

Shift coverage including part of the on‑call team.

Provide field‑based services as required by program and client need.

Maintain confidentiality of client’s care.

Participate in scheduled program meetings.

Other relevant duties as assigned.

Conduct extensive liaison with community housing partners.

Manage maintenance requests and coordinate with vendors for repairs.

Review and manage housing budget, invoice payments, and subsidies.

Maintain tracking systems and complete and submit housing reports.

Qualifications

High School Diploma or GED and at least three years of experience providing housing‑related social services required.

Valid CA driver’s license and current auto insurance per contract requirements required.

Knowledge of housing needs of individuals with behavioral health needs preferred.

Cultural competency understanding required.

Bilingual preferred.

Knowledge of Microsoft Office and EHR software preferred.
