Lead Housing Case Manager – Mental Health Systems Lead Housing Case Manager role at Mental Health Systems (also known as TURN). The position focuses on client-centered care in a purpose-driven environment.

Salary Base pay: $36.00/hr – $38.00/hr (actual pay based on skills and experience).

Job Summary Provide case management with crisis intervention, discharge planning, group & individual work, mental health/substance use counseling, and assistance in meeting basic needs. Coordinate with physical health providers for whole-person care and advocate for clients.

Responsibilities

Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use counseling, screening, and assist clients in meeting basic needs and symptom management.

Complete accurate and timely documentation and meet minimum billable requirements for direct client services if contractually required.

Facilitate placement including pre-placement visits and transportation as needed.

Provide shift coverage as part of the on-call team.

Provide field-based services as required by program and client need.

Maintain confidentiality of client’s care.

Participate in scheduled program meetings.

Other relevant duties as assigned.

Qualifications

Master's degree or bachelor's degree with four years of experience in social services.

For SUD CM, a certification from a recognized board is preferred and registration required.

For Housing, two years of experience and an AA degree in social service or related field or high school diploma and five years’ experience.

Valid CA driver’s license and current auto insurance per contract requirements.

Experience providing mental health and/or substance use disorder services preferred.

Cultural competency understanding required.

Bilingual preferred.

Knowledge of Microsoft Office and EHR software preferred.

Physical Requirements These physical demands represent what must be met to successfully perform the essential functions of this position:

Seeing, hearing, speaking.

Stooping/bending, working in cramped spaces, moving around facility and between offices/clients.

Driving – light errands, driving in the course of job duties.

Staying for long periods, sitting for long periods.

Lifting/carrying heavy items (optional). Climbing, pushing/pulling/dragging items, working outside/underground not required.
